The Clinical Research Of Locking Compression Plate In Treatment Of Tibial Plateau Fracture

【Objective】through exploring the application of locking compression plate in treatment of tibial plateau fracture of the surgical techniques,and anatomy and general support plate in the treatment of tibial plateau fractures compared the efficacy, assessment of locking compression plate in the treatment of tibial plateau fractures clinical value.【Methods】General information:collect and selected 52 cases of tibial plateau fracture inpatients in Hubei Hospistal of Traditional Chinese Medicine and Wuhan General Hospital of Guangzhou Military Region from March 2005 to January 2008.Male 34 cases,female 18 cases,from age 23 to 67,average age 39.6±12.4 years old.23 cases of car accident,21 cases of falling injury, 8 cases of hard object hittigdirectly.In order of comparison,all patients were no nerve,vascular or other soft tissue injury,open fractures and pathological fractures were without the scope of the information,patients with no systemic diseases such as diabetes. Treatment:(1) Incision options:SchatzkerⅠ-Ⅲtype,Ⅴ-Ⅵtake anterior lateral incision;Ⅳmedial knee incision type used. (2) Locking compression plate briefly the process of operation: surgical approach along the above step by step into the exposed joints,exposure of tibial plateau articular surface,prise collapse of articular surface,there were bone defects implanted cancellous iliac bone pad flat articular surface,Ⅰ-Ⅲ,Ⅴ,Ⅵselected fracture lateral tibial locking plate placed in a fixed lateral support;Ⅳselected fracture medial tibial locking plate placed inside a fixed support.(3) non-locking plate briefly the process of operation:surgical approach along the above entry,meniscus-shaped transverse incision under the joint capsule,showing fracture and articular surface,under the prying open fracture reduction so that the tibial plateau articular surface to maximize anatomical reduction,bone of bone defect autologous iliac bone graft.Ⅰ-Ⅲ,Ⅴ,Ⅵlateral fracture in the platform to use "L" shape,Ⅳfracture inside the platform to use "T" to support non-locking plate fixation of fracture.After surgery treatment:prevention of infection after routine,to stop bleeding and promote fracture healing,such as medication, limb early postoperative functional training trip.【Results】Statistical methods:collect two groups of patients operation time,hospitalization period,blood loss volume,the reduction in areas such as loss of data and efficacy of tibial plateau fracture assessment efficacy criteria.All cases the clinical healing of fracturesbe followed up for two weeks to one year.After statistical analysis,two groups of patients at operation time,hospitalization time,blood loss was no significant difference,so that the two treatments in general no significant difference between recovery.Callus at the earliest after the formation of the average amount of time,the average time for fracture healing,the locking compression plate group in support of the efficacy and non-locking plate group no significant difference;in contrast to the first weight-bearing on the average time locking compression plate group advanced to non-locking plate group;in after six months follow-up,the functional recovery of the overall assessment of the knee locking compression plate group in support of the efficacy and non-locking plate group no significant difference;in after 1 year follow-up,locking compression plate group more effective than non-locking plate group effect.【Conclusion】Through statistical analysis,the study group:the structure of locking compression plate unique design,the concept of scientific thinking advanced significantly superior clinical efficacy,and its operation process was simple,operation time, blood loss and a relatively small amount of bone in the maintenance of client angle stability and to avoid periosteal blood supply and soft tissue damage,fracture healing to create a biological environment and the promotion of early knee joint function and to improve the overall long-term knee function recovery rate has broad application prospects and far-reaching value.
