Study On Blood Serum IL-1 Gallbladder Tissue ICAM-1 IL-1 Change With Choledoch Ligation Induced Acute Cholecystitis

Objective: we can observe cholecystitis with obstruction of common bile duct at clinic. But, there are not animal experimentation about cholecystitis with obstruction of common bile duct and blood serum IL-1 gallbladder tissue IL-1 ICAM-1 change. Our study developed the animal model of choledoch ligation. To observe gallbladder pathological, blood serum IL-1,gallbladder tissue IL-1 ICAM-1 change before and after the choledoch ligation. Then we hoped that we can understand this disease mechanism deeply and offer the theory support for cure cholecystitis induce by obstruction of common bile duct at clinic.Methods: 54 healthy rabbits weighing 2.2-2.5Kg were randomly assigned into 9 groups equally. The normal control group (n=6), the group of choledoch ligation one day(B1) (n=6), the group of choledoch ligation three day(B3) (n=6), the group of choledoch ligation five day(B5) (n=6), the group of choledoch ligation seven day(B7) (n=6), the group of relieving ligation after choledoch ligation one day(R1) (n=6), the group of relieving ligation after choledoch ligation three day(R3) (n=6), the group of relieving ligation after choledoch ligation five day(R5) (n=6), the group of relieving ligation after choledoch ligation seven day(R7) (n=6). All the rabbits were prohibited to eat but not prohibited to drink before operation. After building animal model succeedly , the rabbits in the group of choledoch ligation were killed after choledoch ligation one day, three day, five day and seven day. The relieving ligation group animals were killed after relieving ligation three day. The following studies were performed in every group,sterile conditions, blood serum samples testing IL-1and bilirubin ,gallbladder tissue samples testing IL-1and ICAM-1. The gallbladder pathologic changes were observed with light microscope after HE staining. The experimental data was expressed as mean±standard deviation. The statistic significance were analyzed by SPSS 13.0 a statistic sofeware.Results: Animal model were succeely builded because blood serum bilirubin gradually went up with ligation time prolong, and compared with every the group of choledoch ligationthe the data was significant difference(P<0.05). The bilirubin level of relieving ligation groups were reduced significantly than the group of choledoch ligation(P<0.05).Choledoch ligation induced cholecystitis, earlier period of choledoch ligation, B1,B3 group, gallbladder intumesce, gallbladder paries edema, incrassation, membrane plica integrated, light microscope: membrane epidermis integrated, inherent membrane, muscle, serosa edema, blood vessel dilate, congestion, and a few inflammatory cell soakage. Along with ligation time prolong, B5 group gallbladder inflammatory aggravate, gallbladder intumesce, paries become incrassation, bile thickness , part membrane putrescence desquamate, froming small ulceration. light microscope: membrane epidermis desquamate, blood vessel dilate, congestion, membrane, inherent membrane, muscle, serosa have a plenty of white blood cell soakage. B7 group, gallbladder nigrescence, paries become weak, membrane plica disappear, take on putrescene, light microscope: membrane epidermis disappear, a plenty of white blood cell oddment, muscle layer slur, when inflammatory become severity, gallbladder may mortification. Gallbladder tissue ICAM-1 ,IL-1and blood serum IL-1 gradually went up with ligation time prolong, and lifted up to top level in the group of choledoch ligation seven day(B7). Compared with every group of choledoch ligation the date was significant difference(P<0.05). After relieving ligation,the level of blood serum IL-1 and IL-1,ICAM-1 in gallbladder tissue ware reduced significantly than the group of choledoch ligation(P<0.05).Conclusions: 1. Choledoch ligation induce cholecystitis, earlier period of choledoch ligation, gallbladder intumesce, gallbladder paries edema, incrassation, membrane plica integrated, along with ligation time prolong, gallbladder inflammatory aggravate , gallbladder intumesce, paries become incrassation, bile thickness, part membrane putrescence desquamate, froming small ulceration. After relieve ligation, gallbladder inflammatory lighten. 2. After choledoch ligation, the level of blood serum IL-1, bilirubin and gallbladder tissue IL-1 ICAM-1 gradually go up with ligation time prolong. After relieving choledoch ligation, the levels of blood serum IL-1 bilirubin and IL-1 ICAM-1 in gallbladder tissue are reduced significantly than the group of choledoch ligation.
