Reconstruction Of The Anterior Skull Base Defect By Pedicel Frontal Muscle Compound Flap

Objective To evaluate the effect of one stage reconstruction for the anterior skull base defect by pedicel frontal muscle compound flap after cancer removal.Methods(1) Basic research:Common carotid arteries,supraorbital arteries and suprathachlear arteries were dissected and measured in 22 cadaver head and the common carotid arteries were infused red emulsion at the same time;(2)clinical research:Have applied the achievement to clinic,and have established pedicel frontal muscle compound flap restoring the anterior skull base defect,2 patients with single pedicel compound flap,7 patients with double pedicel compound flap,25 patients with double pedicel compound flap and skull, 2 examples with double pedicel compound flap and titanium.The anterior skull base defect area is from 30mm X 30mm to 70mm X 80mm,and the pedicel frontal muscle compound flap area is from 70mm X 100mm to 100mm X 180mm.Results Adopting that method to cure 34 example patients together from the year 1997 to March,2009.Hyperpyrexia appear on 2 patients,and temperature return to normal by the treatment,1 patient had CSF leakage for short time and were cured by lumber puncture and use of antibiotic.30 patients among them get the follow-up of 1 month to 8 years,the rate of follow-up exceed 88%,no complication happened.Conclusion The pedicel frontal muscle compound flap is the ideal method restoring the anterior skull base defect,not only have provided strong tissue,and satisfily closed a cranial cavity having avoided various complication happening.
