Objective:To explore the morbidity situation of fetal congenital malformation.To carry on the chromosome nuclear types of the abnormal fetal to discusses the effecting factors and the necessity of interventional prenatal diagnosis in the level of cytogenetics.To provide reference for consultation before pregnancy,prenatal consultation and genetics consultation according to analysis of the hazard factors of congenital malformation.Methods:Amniocentesis or cordocentesis was gived for fetal karyotype to the abnormal fetal diagnosised by prenatal ultrasound screening in Women and Children Healthcare Centre of Qingdao between July,2007 and Dec,2008. Analy the congenital malformation morbidity situation.In the form of questionnaire survey to the pregnant women,A hospital-based 1:2 matched case control study was designed by means of simple and multivariable conditional logistic regression to calculate odds ratios and 95%confidential interzone of the risk factors.Results:The fetal anomaly detection rate was 2.28%.Most fetus malformation were found between 20 and 28 weeks of pregnancy,which is 41 case among all 60 cases.The times of abnormal phylogeny altogether is 88,Top three of which is congenital heart disease(23 cases),nervous system malformation(18 cases),osseous system malformation(11 cases).After Amniocentesis or cordocentesis prenatal diagnosis of the 60 malformatal fetus,there were 7 examples with unusual chromosome,respectively 18-trisome syndrome(4 cases),13-trisome syndrome(1 case),Turner syndrome(1 case),Down's syndrome(1 case).Multivariable conditional Logistic regression showed that,the main effective factors of embryo Congenital malformation were:catching a cold or having a fever during the pregnant period(OR=6.959),infections with viruses (OR=23.261),getting diseases and taking medicine during pregnancy (OR=5.380),stress of the work(OR=9.764),living in rural area(OR=4.921), Hereditary history(OR=2.754),adverse delivery history(OR=2.297 ),purchase of furniture and home decoration(OR=4.883),explore to toxic substances in work place(OR=9.389).The protective factors were intake of folic acid (OR=0.125).Conclusions:Congenital heart disease and nervous system malformation ranked top among the Congenital malformation.The practical value of ultrasound screening in prenatal diagnosis is much higher,so all pregnant women should take normative ultrasound screening.Some fetal malformation or Mic-configuration abnormity have relationship with chromosome abnormity,interventional prenatal diagnosis should be carried on if necessary.The main risk factors of fetal congenital malformation was catching a cold or having a fever during the pregnant period,infections by viruses,getting diseases and taking medicine during pregnancy,stress of the work,living in rural area,purchase of furniture and home decoration,explore to toxic substances in work place.Intake of folic acid,strengthening premarital care,enhancing pregnancy healthcare,expanding prenatal filtration and taking prenatal diagnosis in time were all effective measures for decreasing the congenital malformation.
