Multility On Prognosis In Patients With Acute Myocardial Infarction

Objective:Comparative analysis the five-year prognosis(divided into survival groups and cardiac death group) in patients with acute myocardial infarction(AMI).Observation myocardial infarction site,infarction type,BP(including SBP,DBP,PP,MAP),admission blood glucose,lipids(including TC,TG,LDL-C,HDL-C),markers of myocardial injury (CK,CK-MB,cTnI),heart rate,complications(including cardiac death,heart failure,arrhythmia,pulmonary infection),age,sex,smoking history,drinking history,EF etc. indicators and the impact of treatment on prognosis.In order to investigate the treatment strategy in patients with AMI and provide a basis to raise the level of treatment.Methods:Retrospective analysis.Collected information of patients who were hospitalized in Shanxi Medical University First Hospital from January 2002 to December 2003 and check the heart of discharge diagnosis for internal medicine and "AMI" information of the cases a total of 198 cases of patients.Followed up by telephone and by letter,the prognosis of the outcome of five years(divided into cardiac death and survival),excluding unfollow-up and non-cardiac death in 14 cases,selected a total of 184 cases of clinical data.Divided into ardiac death group and survival group in accordance with the prognosis of patients.Comparative analysis of two groups of patients with myocardial infarction site,infarction type,BP(including SBP,DBP,PP,MAP),admission blood glucose,blood lipids(including TC,TG,LDL-C,HDL-C),markers of myocardial injury(CK,CK-MB,cTnI),heart rate,complications(cardiac death,heart failure,arrhythmia,pulmonary infection),age,sex,smoking history,drinking history,EF etc.indicators and the impact of treatment on prognosis.Results:(1) Single-factor analysis showed that age,sex,hypertension,diabetes,smoking,alcohol consumption,blood lipids,cTnI abnormal,STEMI,anterior wall myocardial infarction,AMI complicated with cardiogenic shock,heart failure,arrhythmia,pulmonary infection,hemoglobin are impact factors on five-years prognosis of AMI.(2) Logistic stepwise regression analysis showed cardiogenic shock,heart failure,cTnI,age,alcohol consumption and diastolic blood pressure are impact factors on five-years prognosis of AMI.Conclusion:The age,sex,hypertension,diabetes,smoking history,drinking history,blood lipids,cTnI,STEMI,anterior wall myocardial infarction,AMI complicated with cardiogenic shock,heart failure,arrhythmia,pulmonary infection and hemoglobin are impact factors on five-years prognosis of AMI.
