Relationship Between Helicobacter Pylori CagA, P73 Protein And Gastric Carcinoma

Objective:1.To study the infection of cagA(+)strain of Helicobacter pylori,the expression of P73 protein in gastric antral carcinoma,gastric antral ulcer and chronic atrophic antral gastritis.2.To investigate the relationship between the targets above and the gastric carcinoma.3.To study the infection rate of cagA(+) strain of Helicobacter pylori in different types of Epigastralgia in Chinese medicine.Methods:1.The objects were the patients who visited our hospital firstly during April 2008 to December 2008,whose chief complaints were"epigastric pain".They were diagnosed with "Epigastralgia",according to syndrome differentiation. After pathological diagnosis of gastric mucosa biopsyed during gastroscopy, the objects were divided into three groups:gastric antral carcinoma group, gastric antral ulcer group and chronic atrophic antral gastritis group,40 cases for each group.2.The Hp infection was unitedly measured by the Giemsa' s staining with the fast urease test.3.The cagA was detected by polymerization chain reaction(PCR) in the specimens infected by Hp.4.Immunohistochemical method(the S-P law) was used to detect the expression of P73.5.The objects were divided into different types of Epigastralgia,using four methods of Chinese medicine examination together.6.To analyze the datas by SPSS 13.0. Results:1.The positive rates of Hp of different groups:the gastric antra carcinoma group is 72.5%,the gasric antral ulcer group is 82.5%,which are both higher than the chronic atrophic antral gastritis group 42.5%.The statistical difference is significant.2.The positive rates of Hp cagA of different groups:the gastric antral carcinoma group is 79.3%,which is obviously higher than the gasric antral ulcer group 45.5%,the chronic atrophic antral gastritis group 35.3%.The statistical difference is significant.3.The positive rates of P73 of different groups:the gastric antral carcinoma group is 62.5%,which is obviously higher than the gastric antral ulcer group 32.5%,the chronic atrophic gastritis group 27.5%.The statistical difference is significant.4.The average positive degree of P73 of different groups:the gastric carcinoma group is.57.0400±20.9831,which is obviously higher than the gastric antral ulcer group 36.3846±16.1840,the chronic atrophic gastritis group 34.4545±13.3669.The statistical difference is significant.5.The positive rates and average degree of P73 of Hp(+) group is respectively 49.4%,49.6667±21.0267,which is obviously higher than the negative group 24.4%,32.9000±10.1154.The statistical difference is significant.6.The positive rates and average degree of P73 of Hp cagA(+) group is respectively 65.9%,54.8966±21.2087,which is obviously higher than the negative group 28.6%,35.7000±17.0753.There was a positive correlation. between Hp cagA and P73.7.The relevance between gastric antral carcinoma,gastric antral ulcer, chronic atrophic antral gastritis and different types of Epigastralgia. The primary types of gastric antral carcinoma are "damp-heat in spleen and stomach" type and "weakness of spleen and stomach" type,accounting for 37.5%, 30.0%respectively;The primary type of the gasric antral ulcer group is "damp-heat in spleen and stomach" type,accounting for 52.5%.The primary type of the chronic atrophic antral gastritis group is "incoordination between liver and stomach" type,accounting for 35.0%.The statistical difference is significant. 8.The positive rates of Hp of different types of Epigastralgia:the "damp-heat in spleen and stomach" type is 97.7%,which is significantly higher than other groups.9.The positive rates of Hp cagA(+) of different types of Epigastralgia: the "damp-heat in spleen and stomach" type is 83.3%,which is significantly higher than other groups.The infection of cagA(+) strain of Helicobacter pylori or the excessive expression of P73 protein possibly plays an important role in the development of gastric carcinoma;The assessment of cagA gene and the P73 protein is helpful to prevent and cure gastric cancer.The primary type of Epigastralgia in Chinese medicine is "damp-heat in spleen and stomach" type, which is accompanied by Hp and susceptible to gastric ulcer or gastric cancer. It is advised that patients of this type should be gastroscopied and eliminated from the infection of Hp.
