Objective1.To investigate the current condition of the early warning and response system and its capability in dealing with public health emergencies in Zhangjiagang.2.To explore the evaluation index for establishing urgent response system of public health emergencies.3.To lay foundation for the enhancement of the system's urgent response and handling capability.4.To provide basis for the formulation of evaluation standard for the urgent response system of public health emergencies.Methods1.Sampling methods: Multistage stratified random sampling method was used. 3 general hospitals and 1 CDC in urban district and 3 service centers for health management in 3 towns were randomly sampled as investigation spots from whole city and 9 towns. In each investigation spot, administrative management section, on-the-spot rescue section, on-the-spot emergency handling section, laborotary section and information section, etc. were used as the sampling units and enough investigation objects were enrolled.2.Investigation methods: Unifiedly designed questionaire for each department and its staff were used in CDC and general hospitals in urban district and towns, which include basic situation, forecast and precaution, monitoring and control, education and training, laborotary situation, information collection and analysis, rapid reaction capability, emergency rescue and medical aid, and the integral state of the emergency's handling. Meanwhile, a group of 8 people with same or similar background and career was led by host to start the discussion about an issue of general interest. The discussion which last 1.5~2.0 hours was voice- recorded by recording pen and words-recorded by a recorder. The individual interview was performed with the chiefs Health Bureau and CDC, which include disease monitoring, early warning and response system, responsive ability, existing problems, etc. The information above was sorted out and into literal materials after the interview.3.Statistic methods: Epidata 3.1 and SPSS softwares were used for data inputting and analysis. Contentanalysis was used in group discussion and individual interview. The material was classified into different topics and categories. In each step of pressing the material, the information would be reread to avoid missing any information.Results1.The information management systems for the reports of public health emergencies in each department were well established and the internet-based network for reporting monitoring information on dread diseases also run smoothly. The diseases that all the medical and health departments could handle better are hepatitis, cholera, flu, and food-borne diseases. Some diseases or events, such as plague, anthrax, unknown cause disease, occupational poisoning, radiation exposure, and natural disaster, are what they could not manage well. They all have no idea of how to deal with bioterrorism events.2.Most of the departments showed strong ability in dealing with ordinary diseases or emergency and could recognize the routine pathogenic agents and substances, except virus and pathogen of unknown disease. The medical departments have weakpoints in the ability of collecting and analyzing the monitoring information.3.The staff from CDC have a higher awareness rate on knowledge related with public health emergency, which is up to 100% . Except the knowledge about Law on Prevention and Treatment of Infectious Disease and Regulation on the Urgent Handling of Public Health Emergencies, the medical staff from medical departments got a relatively lower rate, which is 53.3% to 73.3%, on other aspects. Conclusion1.The early warning and response system of public health emergencies in Zhang Jiagang were preliminarily established and still need to be advanced and improved. Each department showed strong ability in dealing with ordinary diseases or emergency, but weak in coping with plague, anthrax, congregative unknown disease, serious occupational poisoning, chemical leakage, radiation exposure and bioterrorism events. The labs'comprehensive capability in detection still need to be raised.2.The government need to increase the invest in the field of public health, establish systematical laws and regulations, construct centralized commanding system, perfect information net-work and disease monitoring. To define and bestow the corresponding function to CDC and medical departments in dealing with emergency. To advance the early warning, monitoring and reports of public health emergencies and ensure a good preparation.
