Study Of Endovascular Treatment For Intracranial Aneurysms And Follow-up

Objective:To summarize the clinical experience of endovascular embolization for intracranial aneurysms,evaluate the near and long-term efficacy of intracranial aneurysms in term of the result of follow-up and discuss the influential factor of aneurismal recurrence.To institute rational therapic strategy.Methods:79 cases of intracranial aneurisms which had been treated by the endovascular therapy with clinical data,the way of embolism,treatment results, clinical and imaging follow-up results were retrospectively analysed.Results:Among 79 cases of intracranial aneurysms(totally 87 aneurysms),2 cases were failed.After endovascular treatment,instantly angiography demonstrate that 48 aneurysms(56.5%) were totally embolized,30 aneurysms(35.3%) were near-totally embolized,7 aneurysms(8.2%) were partly embolized.Clinical follow-up was graded using the modified Rankin score(RS) and revealed 63 patients(79.7%) in the RS 0 category,9 patients(11.4%) in the RS 1-2 category,4 patients(5.1%) in the RS 3-5 category,3 patients(3.8%) in the RS 6 category.Follow-up imaging performed on 61 cases(63 aneurysms) for periods ranging from 1 month to 4 years, Follow-up result demonstrate that 54 aneurysms(85.8%) were not recurrent,9 aneurysms(14.2%) were recurrent.Incomplete embolization of intracranial aneurysms were with a high recanalization rate.It is difficult to densely embolize wide-necked aneurysms.The recanalization rates of wide-necked aneurysms were higher than those of narrow-necked aneurysms.Conclusion:Endovascular embolization is a minimally invasive,safe,effective modality for the treatment of intracranial aneurysms.Dense and complete occlusion is a primary principal to prevent the recurrence and rerupture of aneurysms.Intracranial aneurysms with endovascular treatment and follow-up programs should be personalized.
