Treatment Of Intra-hepatolithiasis By Choledochoscope And Electrohydraulic Shockwave Lithotripsy

Objective:To observe the efficancy of Choledochoscope and Electrohydraulic Shockwave Lithotripsy in treating intra-hepatolithiasis,analyzes their indications and complications,to improve the treatment of hepatolithiasis effect,reduce the recurrence and vestigital stones.Method:From January of 2007 to October of 2008,43 cases with Intra-hepatolithiasis were treated with choledochofiberscopy and Electrohydraulic Shockwave Lithotripsy in our department.All the cases had different degree of abdominal pain,cholangitis,hypohepatia,jaundice,etc,which combined 1 case liver cirrhosis,3 cases with diabetes mellitus.Result:43 patients were treated with choledochoscope,26 cases were treated with Electrohydraulic Shockwave Lithotripsy,33 cases were treated with per-operative choledochofiberscopy,10 cases were treated with post-operative choledochoscope.21 cases underwent eholangioenterostomy and T-Tubeinsertion,7 cases underwent hepatectomy,5 cases underwent choloenteroanastomosis,40 cases were success,3 cases were facure.The success rate was 93.02%,and the failure rate was 6.98%.39 cases were visited by telephone,the return rate was 90.7%Conclusion:Choledochoscope and Electrohydraulic Shockwave Lithotripsy can detect gravel stones,expansion narrow,treatment and diagnosis in treating Intra-hepatolithiasis. Choledochoscope and Electrohydraulic Shockwave Lithotripsy are very effectual in treating Intra-hepatolithiasis,can significantly improve operation efficiency,and reduce residual stones.
