Objective:Initially to explore the value of Dynamic Optical Breast Imaging compared with ultrasound in differentiating benign breast lesions from malignant ones.Materials and Methods:A total of 74 usable breast lesions in 76 patients were examined with the Duplex ultrasound and DOBI,also the sonograms and DOBI were analyzed respectively.The cases of 74 were divided with three groups according to maximum diameter(<1cm 1~2cm>2cm).The results were compared with histopathology in ultrasound and DOBI in differentiating benign breast lesions from malignant ones.Besides the 74 cases were divided with two groups according to maximum diameter,the cases of first group were small breast lesions which measure of ultrasound were≤2cm;the cases of second group were>2cm. The breast lesions of two groups were diagnosed in ultrasound combined with DOBI,then the results were compared with histopathology in ultrasound and ultrasound combined with DOBI to differentiate breast lesions.The blue area in DOBI image pointed suspicious pathological changes (outside of nipple),Discriminating benign breast Lesions from malignant had four diagnosis standard of different image in DOBI,(1)The target appeared location(2)The target appeared change in space following time(3)The percentage of the brightness in the target changed with time(4) The curve in target compared with the curve in normal area.Then these breast lesions were differentiated benign from malignant by ultrasound in association with DOBI,the criterion of associated diagnosis:the results of associated diagnosis were benign,when the results were benign in ultrasound Or DOBI respectively,those results of malignant were same.The results of diagnosis were malignant according with each hereinafter,when the results had not sameness in ultrasound or DOBI.(1) The curve of target in DOBI declined linear or the range of declining was>—4 in 20s';(2) The frontier of lesions was abnormality in ultrasound;(3) The results were dubious in DOBI or ultrasound(The frontier of lesions were abnormality partly and were not envelope).The results of diagnosis were benign according with each hereinafter:(1) The blueness did not appeared in target;(2) The lesions were vesica or had integral envelope in ultrasound.ResultsThe number of diagnosis in every group was 22,31 and 21.The respective sensitivity,specificity and accuracy of diagnosis with DOBI were 85.7%,80% and 81.8%in<1cm group;the rat of diagnosis with ultrasound were 71.4%,40%and 50%.The sensitivity,specificity and accuracy of diagnosis with DOBI were 78.6%,64.7%and 71%in 1~2cm group;the rat of diagnosis with ultrasound were 85.7%,76.5%and 80.6%.The sensitivity,specificity and accuracy of diagnosis with ultrasound were 83.3%,77.8%and 81%in>2cm group;the rat of diagnosis with DOBI were 25%,44.4%and 33.3%.Besides the sensitivity,specificity and accuracy of diagnosis in ultrasound combined with DOBI were 95.2%,81.2%,86.7%in<2cm small breast lesions; and the rat of diagnosis in ultrasound combined with DOBI were 83.3%,88.8%,85.7%in>2cm breast lesions.There was no significant difference between DOBI and ultrasound for differentiating benign from malignant breast lesions(P>0.05).The sensitivity,specificity,accuracy of diagnosis with DOBI were higher than those of ultrasound in first group(P<0.05);There was no difference of diagnosis in DOBI and ultrasound in second group(P>0.05);The sensitivity and accuracy of diagnosis with ultrasound preceded diagnosis with DOBI in third group(P<0.05).The rate of diagnosis in ultrasound combined with DOBI were higher than those of conventional ultrasound in≤2cm small breast lesions(P<0.05);The specificity,accuracy of diagnosis in ultrasound combined with DOBI were a little higher than those of conventional ultrasound in>2cm breast lesions(P>0.05).ConclusionsThere was no significant difference between DOBI and ultrasound for differentiating benign from malignant breast lesions;the rate of accurate to differentiate breast lesions with DOBI were related to the size of breast lesions; DOBI could help to diagnose and differentiating diagnose breast lesions which were atypical characters in ultrasound,especially which maximum diameter were<1cm.Ultrasound combined with DOBI could enhance accuracy of differentiating lesions evidently in≤2cm;and could better avoid leaking and mistaking to diagnose small breast lesions.
