Abstract/Summary:
It is well known that cardiovascular disease(CVD) especially eoronary heart disease(CHD) has become the world's number one cause of death and disability in 21st century which thanks largely to the transition of hurman disease Profile and world economic development .In China, with the improvement of people's life, coronary heart disease had become the major cardiovascular disease which harmed to the aged people. Acute coronary syndrome is a serious clinical manifestations of coronary heart disease, and compared with other types of coronary heart disease, it showed incidence of acute and severe symptoms, poor prognosis and high mortality for severe acute coronary heart disease, a serious threat to people's health .How would you assess the severity of ACS and how to predict its development and prognosis, have become the most pressing problem for clinicians . Creatinine clearance rate refers to the time put nephron number or plasma creatinine clear out all. Are currently the most commonly used clinical glomerular function tests, it can reflect the early glomerular dysfunction and estimate the extent of damage. Recent study at home and abroad have shown that creatinine clearance rate (Ccr) in patients with acute coronary syndrome and prognosis of early complications in the prediction has an important role.Purpose: Discussion the prognosis of patients with Ccr of ACS and the impact of changes in Ccr in patients with ACS. For the future can be judged to provide a clinical prognostic indicator ACS possibilities.Mothod: We collect a total of 160 patients with ACS as the research objects, and According to the calculation formula Cockcroft-Gault income groups: according to the Ccr values divided into normal group (Ccr≥70 ml/min) and Ccr reduce group (Ccr<70 ml/min), According to the Ccr reduced level turns into A group (degree of 70ml/min>Ccr≥50ml/min) and group B (Ccr<50ml/min). All patients had ECG, blood, urine and other routine checks. Except these, hypertension, diabetes and other risk factors were analyzed.Results:Three groups of patients in gender,smoking,ECG changes in chest pain when,high blood pressure,Hpertension,diabetes and blood serum uricacid in the index by statistical analysis, no significant difference (P > 0.05).According to the Ccr level grouping, Ccr reduce group of 74 cases, has 44 cases with complications, accounting for 59.46%.Ccr normol group of 86 casess who have complications 32 cases, accounting for 37.21%. Two groups of patients the incidence of complication was significant differences (P<0.01), the incidence of complications in patients of Ccr reduce groups are higher than normal group's.Will the Ccr<70ml/min patients were divided into 70ml/min>Ccr≥50ml/min(n=38) and Ccr<50ml/min(n=36) with two parts of the single factor analysis, Two groups of patients the incidence of complication was significant differences .Display that the incidence of heart failure and arhythmiaetc of Ccr<50 ml/min patients are increased significantly than 70ml/min> Ccr≥50ml/min patients.Various clinical variables to analysis the influences of reduced Ccr specific situations, age>75 year patients with 51 patients of reduced Ccr group,40 patients of Ccr normal group. Three dieases patients with 43 patients of Ccr lower group, 27 patients of Ccr normal group, there were significant differences. Statistical analysis showed Ccr reduced group by age> 75 year patients and multivessel coronary lesions the proportion accounted for more than Ccr normal group(P<0.01).Conclusion:1.Those findings demonstrate that the patients with Ccr<70 ml/min in ACS will have a higher incidence of complications.2.Ccr<50ml/min patients in hospital with heart failure and the incidence of heart attacks are increased significantly than 70ml/min>Ccr≥50ml/min patients.3.Ccr may predict short-term and long-term prognosis in p atients with acute coornary syndormes.4.More vessels coronary disease and age>75 year can affect the Ccr of patients .5.Ccr is a important factor to effect the prognosis of patients with ACS .Intervention on renal function may be conducive to enhance the protection of its survival rate, clinical yet to be confirmed by further studie.
