Abstract/Summary:
Background:With the rapid development of traffic, construction, sports , and the speeded-up of industrialization, more and more cases of cervical spine have been seen. Among them, the Unilateral locked facet has lower incidence, light symptoms, and can be easily overlooked, resulting in misdiagnosis. Lower cervical unilateral locked facet is largely caused by Traffic injury, fall injury, sports injury or violence. Unilateral cervical locked facet is Rare than bilateral and has less Neurological complications. The incidence of it Accounts for 3% -11% of cervical spine injury. The performance of a simple injury are only partial symptoms of the neck. Patients with Nerve root irritation or oppression shows skin allergies, pain or declining feeling in regions of Nerve root distribution. The performance of the Injured Spinal Cord incorporates corresponding Quadriplegia, paralysis of lower limbs or partial paralysis. Through the lateral X-ray film, CT, MRI examination, the characteristics of such change is not difficult to make a diagnosis, and can be a guidance for the treatment. According to the biomechanical study of the mechanism of its injury,Unilateral cervical locked facet is caused by the rotation violence to the cervical vertebra. Cervical stability, morphological damage to the spinal canal, and spinal cord injury are subject to different degrees. Among them, violence of Flexor rotation is the most common reason. Unilateral locked facet can lead to the destruction of the continuity of Cervical spinal and mechanical compression of spinal cord and blood vessels. In addition to the direct damage early, if improperly treated, Post-secondary spinal cord injury can be the main reason for dysfunction with the development of medical devices, bio-mechanics research progress, rising the level of surgery. At present, the treatment for the unilateral locked facet has made significant improvements. Conservative treatment are reported to be likely to add to spinal cord injury, deteriorate neurological function,and has an unfavourable effect for the prevention of long-term complications. Most scholars advocate early surgical treatment to lift oppression,and it indeed helps restore nerve function of spinal cord. However, there are still some controversion about the choice of surgical procedures. According to preoperative CT, MRI and clinical symptoms,the major procedures are:â‘ the anterior approach open reduction and internal fixation operation;â‘¡the posterior approach open reduction and internal fixation operation;â‘¢the posterior-anterior approach open reduction and internal fixation operation;â‘£the anterior-posterior-anterior approach open reductionand internal fixation operationThese Operations provide valuable clinical experience and guidanceIt has been reported in the literature, some patients with Lower cervical unilateral locked facet can show intervertebral disk hernia and oppression of dura mater in MRI and CT.And varying degrees of disc injurycan be seen in all patients with lower cervical unilateral facet in the corresponding segment during the operation. In this paper, through Corresponding discography in patients with unilateral locked facet,and the C-arm X-ray machines under the perspective, the situation of disc can be more intuitively displayed. According to whether or not cervical disc is injured and whether the stability of cervical spine is damaged, Advantages and disadvantages of different operations of Lower cervical unilateral locked facet is analyzed from the view of stability of cervical spine."the anterior approach open reduction and internal fixation operation"is highlightened.Objective:To prove the anterior approach operation is necessary to the treatment of the Unilateral Locked-Facet Joint in Lower Cervical Spine . Methods:To visualize the homologous Intervertebralis of Unilateral Locked-Facet Joint in Lower Cervical Spine in patients who is hospitalized in China-Japan Union Hospital of Jilin University during October.2008 to March. 2009.To analyze whether the homologous Intervertebralis is injured or not,and the stability of the Cervical Spine is destroied or notResults:The homologous Intervertebralis of Unilateral Locked-Facet Joint in Lower Cervical Spine is indeed injured, and the stability of the Cervical Spineis indeed destroied .Conclusion:In order to recover the stability of the Cervical Spine ,the anterior approach operation is necessary...
