Objective Compare the safety,the efficacy and the superiority of laparoscopic versus open approach to liver resection. Methods A retrospective study was made on the clinical results of 14 cases of laparoscopic hepatectomy(LH)and other 19 cases of open hepatectomy(OH)in the First Hospital of Jilin University from January 2007 to December 2008.Result The data of LH group was significantly less than that of OH group in the mean length of incision,the mean postoperative abrosia time,the mean dose of the analgesics,the mean postoprative hosptal stay (P<0.01),the mean operation time,the mean time of drainage and total hospital stay(P<0.05).The mean operation cost of LH was higher than OH(P<0.01).There were no significant difference of the mean intraoperative blood loss and the total cost between two groups(P>0.05).There was 1 patient occurred postoperative blood loss in LH group,while there was 1 patient occurred postoperative blood loss and pleural effussion,and 1 patient occurred infection of incisional wound.There were no patients who died in the hospital.Conclusions In selected patients,LH is a safe and effective approach to liver resection. Comparing to OH,LH delivers the benefits of decreased blood loss, shorter the time of drainage and hospital stay,lesser requirement for analgesics,and an earlier return to oral diet。LH is an effective treatment which can replace the OH completely in selected patients.
