Purpose: To prospectively evaluate accuracy of 64–detector row computed tomographic (CT) images for preoperative staging of gastric cancer by using surgical and histopathologic results as reference standards.Materials and Methods: Fifty-four consecutive patients with advanced gastric cancer (41men, 13 women; age range, 39–79 years; mean age, 62 years) underwent preoperative CT. Before enhanced CT scanning for gastric distention Patients drank 600 mL of water to enable gastric distention.CT scans Images were obtained in late arterial, venous. All patients underwent surgery. CT findings were compared with surgical and histopathologic results. Whether these patients according to N2 lymph node metastasis in CT scans Images were divided into two groups.The one group which have evidence of N2 lymph node metastasis in CT scans were 30 patients. The other group which have no N2 lymph node metastasis in CT scans were 24 patients. Compared two groups of patients with that the respectability of tumors and the extent of lymph node dissection.Result: CT in the pre-operative determination of depth of invasion (T stage) accuracy were:T2 90.9%,T3 67.7%,T4 83.3%. The sensitivity of N2 lymph node metastasis was 93.5% and specificity of 96.0%; P> 0.05, the difference was not statistically significant. CT scan showing the assessment of patients with N2 lymph node-positive tumors can be radical resection rate of 66.7% accuracy (21/30). Assessment of patients with N2 lymph node-negative tumors can be radical resection rate was 91.6% accurate (24/24).Total assessment of all patients resectability rate was 100% accurate. Compared two groups of patients with radical surgery is the number of cases P=0.046, statistically significant difference between the two.Conclusion: 64–detector rowCT can accurately determine the around organ invasion in patients with T4 gastric cancer . CT scans have the high accuracy and sensitivity of N2 lymph node metastasis. 64–detector row CT is a promising method for pre-operative evaluation of advanced gastric cancer.
