ObjectiveTo identify some clinical and laboratory parameters with prognostic value in relation to survival of patients with chronic lymphocytic leukemia(CLL),and to summarize the common complications of CLL.Methods203 cases of CLL patients were reveiwed and followed up.Survival analysis(Kaplan-Meier analysis,log-rank test and COX regression model)was used to analyse the prognostic factors,and the common complications of CLL were also summarized.ResultsThe median follow-up time was 48.0(3.0~156.0) months,the five-year overall survival rate was 87.3±2.4%,the ten-year OS rate was 77.4±3.3%,Forty-eight(23.6%) patients died.The univariate analysis indicated that advanced clinical stage, B symptoms,extranodal involvement,number of lymphoid-bearing areas≥3,enlarged liver,Hb<100g/L,BPC<100×10~9/L,absolute lymphocyte count(ALC)>50×10~9/L,atypical morphological type,progression in clinical stage,non-responsive to treatment,complicating secondary cancers or transformations were associated with poor prognosis.Multivariate COX analysis indicated that lymphoid-bearing areas≥3 and atypical morphological type were independent poor prognostic factors. The common complicatios in CLL included infections,autoimmune disease(AID) and secondary cancers.Cytopenia caused by AID was not an adverse prognostic factor.ConclusionThe number of lymphoid-bearing areas and morphological type are useful for assessing the prognosis.Without the new evaluable prognostic factors,the patients who were in early clinical stage and without therapeutic indication should adopt "watch and wait" policy until progression.New prognostic factors should be further explored.
