Objective: To investigate the changes and clinical significance of hyaluronate (HA) level in serum and synovial fluid (SF) from patients with osteoarthritis (OA) in different X-ray grade. Methods: The HA level of serum and SF was measured by a highly specific radio-immunological assay in 80 patients with OA,20 normal. Serum and SF samples from donors without joint involvement. Results: The HA levels of SF in OA patients were significantly decreased compared with normal SF (P<0 01) whereas the HA levels in serum were increased compared with normal serum (P<0 01). A lower ratio of SF HA/serum HA was found in patients with generalized OA or with knee radiohraphic grades 3 and 4 (Kellgren and Lawrence grade ). Conclusion: Serum and SF HA may be unique as an inflammatory marker of OA, The ratio of SF HA/serum HA is perhaps a reflection of synovial involvement and inflammation in patients with OA.
