The Clinical Research On Nuclear Factor-Kappa B,TXB2,6-Keto-PGF1a In Children With Frequent Relapse Nephrotic Syndrome By Treatment Of Decoction Shenkangling By Tonifying The Kidney And Promoting Blood Circulation

Objective:To observe the intervention in children with frequent relapse nephrotic syndrome(FRNS) by treatment of decoction shenkangling by tonifying the kidney and promoting blood circulation,to discuss the correlation of thromboxane B2(TXB2),6-keto-PGF1αand FRNS and kidney asthenia with blood stasis syndrome,to search the objective index of the recurrence and prognosis and TCM syndrome differentiation of children with FRNS。Further to explain the pathogenesis of FRNS and the mechanism of Shenkangling,and meanwhile to provide remission rate,decrease recurrence rate,reduce side effect for clinical treatment in children with FRNS.Methods:68 children patients with FRNS from Traditional Chinese Medical College of Fujian and Fuzhou General Hospital of Nanjing Military Region during 2006 and 2007 years were randomly divided into group A(Shenkangling+Westen medicine) with 35 cases and group B(Westen medicine) with 33 cases.30 healthy subjects served as group C.Before the treatment and after three months' treatment,NF-κB,TXB2,6-keto-PGF1αof plasm,urine protein of 24 hours,as well as serum total protein(TP),albumin(ALB),renal function (urea niteogen,creatinine) total cholesterol(TC) and triglyceride(TG) of all patients were tested.Statistical analysis was performed by SPSS 13.0 system software.Results:1.Before treatment,the levels of NF-κB and TXB2 of group A and group B were significantly increased than group C(P<0.01),the levels of 6-keto-PGF1αof group A and group B were significantly decreased than group C(P<0.05).After treatment,the levels of NF-κB and TXB2 of group A and group B were significantly decreased(P<0.01),the levels of 6-keto-PGF1αincreased(P<0.01),group A were decreased NF-κB and TXB2 more significantly than group B(P<0.01),increased 6-keto-PGF1αmore significantly(P<0.05)。2.After treatment,the levels of 24 hours'urine protein,Ch and TG of group A and group B were distinctly decreased,the levels of ALB,TP were enhanced obviously(P<0.01); compared with group B,in group A,both 24 hours'urine protein and TG decreased remarkably(P<0.01),Ch decreased remarkably(P<0.05),ALB and TP enhanced remarkably(P<0.05)Conclusion:1.NF-κB and TXB2 increased,the ratio of TXB2/6-keto-PGF1αdisequilibrated,urine protein,Ch and TG increased,ALB,TP decreased on children with FRNS.2.Decoction Shenkangling with function of "Benifiting the kidney and promoting blood circulation",can remarkably ameliorate the patients'condition and experimental indexes: decrease NF-κB,redress the disequilibrium of the ratio of TXB2/6-keto-PGF1α,reduce urine protein,TC and TG and enhance ALB,TP.3.The clinical situation of the disequilibrium of NEI Network is similar to the Symptom of"Kidney Asthenia and Blood Stasis"in TCM,NF-κB,TXB2,6-keto-PGF1αcan be used as objective indexes of the diagnosis of "Kidney Asthenia and Blood Stasis"in TCM and the applications of"Benifiting the kidney and promoting blood circulation".4.Decoction Shenkangling with function of "benifiting the kidney and promoting blood circulation",can remarkably ameliorate the patients'above experimental indexes,decrease NF-κB,redress the disequilibrium of the ratio of TXB2/6-keto-PGF1α,enhance ALB and TP,reduce urine protein,TC and TG.This may be explained:Decoction Shenkangling can regulate the homeostasis of NEI network.5.The curative effects on children with FRNS By treatment of matching Methods of Decoction Shenkangling with function of "benifiting the kidney and promoting blood circulation"with westen medicine overmatches using only westen medicine at decreasing NF-κB,redressing the disequilibrium of the ratio of TXB2/6-keto-PGF1α。...
