Objective:Rectal carcinoma is one of the most Commonly Gastrointestinal neoplasm in our country and still increasing year by year,which badly do harm to people's health.It is of significant meaning to guide prophylaxis,early discovery and early treatment by understanding the pathogenesis and metastatic patterns of rectal carcinoma.Heme oxygenase-1(HO-1),also called Hot Shock Protein 32(HSP32)is the start enzyme and the key enzyme of resolving heme,that is released by senescent red blood cell and hemoglobin in hemocirculatory,into Iron Ions,biliverdin and carbon monoxide.The three kind of products are the main effect molecule of HO-1 in anti-inflammation,anti-oxadation,anti-Apoptosis and anti-hyperplasia.Toll-like receptor(TLR)is the homologisation of Toll proteins and the innate immune factor discevered and studied prevalently nowadays.TLR plays crucial roles in proinflammatory,promoting immune cells' differentiation and maturation and regulating immune responses.TLR2/4 are two kinds of TLR studied popularly and understand fully.Latest research indicated that HO-1 and TLR2/4 may participate in the generation,development and metastasis of multiple tumors,as well as treating tumors as the immunization adjuvant.In this study,our purpose is to approach the effect of HO-1 and TLR2/4 in rectal carcinoma's occurrence and development to morely research its pathogenesis and treatment by detecting the expression of HO-1 and TLR2/4 in rectal carcinoma tissue and normal tissue by ABC immunohistochemical method and RT-PCR.Methods:30 rectal carcinoma tissues from radical resection of rectal carcinoma were collected by Department of gastrointestinal surgery of Shandong Provincial Hospital from December 2006 to April 2007 as experimental group(male 18,female 12;age:51 to 82,average 64;upper 13,middle-low 17).Aproved by Pathology,the rectal carcinoma tissue should be cutted apart from the tumor brim less than 3 cm, collected within 30 minutes and kept in liquid nitrogen immediately.Normal tissues were collected as control group.The experimental data were expressed with((?)+s) and analysed by SAS 9.13 statistic software.Results:Immunohistochemistry:HO-1 was expressed in both rectal carcinoma group and control group.HO-1 was stained a little in control group,while was strongly expressed in rectal carcinoma group.The difference expression of HO-1 between rectal carcinoma group and control group were significantly(P<0.05).HO-1 mainly expressed in cytoplasm of tumor cell and less in cell membrane.RT-PCR: HO-1 and TLR2/4 mRNA were expressed in all groups.They were more strongly expressed in rectal carcinoma group while feebly expressed in control group.There was significant deviation between rectal carcinoma group and control group(P<0.05).Correlation analysis showed that the expression of HO-1 was significantly correlated with that of TLR2/4(HO-1 vs TLR2:r=0.8272 P<0.05,HO-1 vs TLR4:r =0.9133 P<0.05).Conclusions:HO-1 and TLR2/4 play a key role in the pathogenesis of rectal carcinoma.It was reported presently that HO-1 was related to the apoptosis of tumor cell,while the cytokine released by the developing tumor may activate HO-1. Over-expression of TLR2/4 may induce the generation of cytokine by signaling pathway,which lead to immunological escaping of tumor cell and promote the growth of tumor cell.TLR4 is the key receptor of LPS inducing cell to produce HO-1.Both TLR4 and HO-1 may induce the release of variouse inflammatory factors by MAPK signal pathways,which lead to a series of biological reactions.This study validated that the over-expression of HO-1 and TLR2/4 was well evidenced in rectal carcinoma and they were significantly correlated with each other in signal pathways.It is proved that HO-1 and TLR2/4 may play an important role in the generation and development of rectal carcinoma and provide a new idea of more discussing the pathogenesis and treatment of rectal carcinoma.
