Objective: This study is performed to investigate the effects of human amniotic homogenate on blebs and expression of TGF-β1 (transforming growth factor-β1) , CTGF (connective tissue growth factor) used by topically after trabeculectomy in rabbits ,to explore amniotic homogenate's inhibiting scar after the glaucoma filtering surgery.Methods: Thirty New Zealand White rabbits were randomly divided into two groups: control group and experimental group. Each group consists 15 rabbits(15 eyes). Each right eye was given standard trabeculectomy. After trabeculectomy, human amniotic homogenate and chloramphenicol eye drops four times daily applied topically to the experimental group for 14 days, chloramphenicol eye drops only to the control group for 14 days. We observed the rabbits filtration blebs,intraocular pressure and complication on day 2,5,10,14 after surgery. Intraocular pressure measurement was performed before surgery and on postoperative day 2,5,10,14 respectively. We compared the difference among the two groups on different stages. Following surgery,rabbits were randomly chosen at 5,10 and 14 days for enucleation. We examined five eyes at each point.Their eyes were enucleated. Their filtration blebs were kept and fixed.They were made paraffin slices .We observed the filtration passage scar formation and the number of fibroblasts by performing HE staining. We observed the expression of TGFβ1,CTGF in the filtration area by performing immunohistochemical staining.Results: 1) The chang of intraocular pressure: There was no significant difference among two groups before operation and postoperative day 2(P>0.05). The intraocular pressure of control group is higher than experimental group after 2 days (P < 0.05) .2)The comparison of filtration blebs:The total numbers of functional filtration blebs are 22 eyes, 14 eyes in experimental group and 8 eyes in control group (P<0.05). There was no difference on the percentage of the filtration blebs among two groups on postoperative day 2,but it was higher than the control group postoperative day 5,10,14(P<0.05).3) Complication: No shallow anterior chamber injured corneal epithelium edema,conjunctival leaking wound,detachment of retina,macular edema and other complications were observed. 4) HE staining:All filtration passways were opened on day 5 after surgery. The percentage of opened filtration passage was higher in experimental group than the control group on day 10 and 14 postoperative. The mean number of fibroblasts was lower than those of the control group on day 14 postoperative(P<0.05) .5) Immunohistochemical staining:TGFβ1,CTGF were expressed maximally on day 5 after surgery. There was significantly statistical difference in the expression of TGFβ1,CTGF between the two groups on days 5,10 and 14 postoperative (P<0.05).Conclusion: 1) The results suggest that supernatant of homogenized amnion used by topically has no side effects,and can decrease the introcular pressure after trabeculectomy.2) The supernatant of homogenized amnion has function on inhibiting the proliferation of the fibroblast,improving filtering function,and keeping the filtering passage unobstructed after the glaucoma filtering surgery. 3) The supernatant of homogenized amnion can inhibit the expression of TGFβ1,CTGF after trabeculectomy.
