Objective: To evaluate the efficacy and toxicities of three dimensional-conformal radiotherapy combining with concurrent chemotherapy in non-small-lung cancer .Methods: From September 2006 to December 2007,37 patients with non-small lung cancer were eligible for the analysis. There are 30 males,7 females;27 patients with lung squamous carcinoma, 9 with adenocarcinoma, 1 with adenosquamous carcinoma; 21 patients with stageâ…¢a lesion and 16 patients with stage IIIb lesion.The therapy method used three dimensional-conformal radiotherapy ,radiotherapy regime was 1.8~2Gy per fraction,one fraction per day, the total radiation dose was 60Gy,5 fractions perweek, 30-34 fractions.The plan required 95% total radiation dose included 100% PTV volumes, based on the GTV,CTV expanded 0.6 cm(squamous carcinoma) and 0.8cm (adenocarcinoma) , based on the CTV, PTV expanded 0.5cm, fluctuation expanded 1cm . we gived proper modification near the spinal cord .The whole lung V20 required under 32%,the mean dose under 15Gy,the mean dose of the heart undered 30Gy,the maximum dose of the spinal cord under 45Gy. Chemotherapy method used vinorelbine plus cisplatin, vinorelbine were used 25mg per centiare on days 1 and days 8;DDP were used 75 mg per centiare on days 2 to days 4.One circle includes 28 days, total 3 circles .All pantients completed the radiotherapy,11 patients completed one circle chemotherapy, 3 patients completed two circles chemotherapy,23 patients completed three circles chemotherapy. Toxicity were evaluated during the treatment .After one month of treatment the short time effects were analized and followed.Result: The final follow-up date was on December 31, 2007.The follow-up rate was 100%.There were 8.11%(3/37) patients completing response (CR), 86.49%(32/37) with partly response (PR)were, 5.4%(2/37) with stable of disease (SD) ,respectively.The over response rate (CR+PR)were 94.6%(35/37).Acute radiation-esophagitis was accurred , 3 cases with 0 grade, 11 cases with 1 grade , 29 cases with 2 grade and 14 cases with 3 grade ,without 4 grade patients.Acute radiation-pneumonia was accurred with 0 grade 5cases, 1 grade 17 cases, 2 grade 15 cases, 3 grade 8 cases, There were not 4 grade patients. There were 32 cases with 0 grade , 2 cases with 1 grade, 1 cases with 2 grade, 2 cases with 3 grade for toxiticies of liver and kidney. Hematologic toxicity: there were 3 case with 0 grade hypoleukemia, 11 cases with 1 grade , 10 cases with 2 grade , 12 cases with 3 grade , 1 cases with 4 grade ; thrombocytopenia, 0 grade 30 cases, 1 grade 4 cases, 2 grade 2 cases, 3 grade 1 cases; hemoglobin, 0 grade 23 cases,1 grade 9 cases 2 grade 5 cases.All the pantients had gastrointestinal tract reaction of them , 14 patients was observed vomiting , 1 patients diarrhea, 13 patients constipation.1-year survival rate were 75.78% . By logrank test,clinical stage ,the weight drop before treatment showed significant difference on 1-year survival rate .Conclusion :Three dimensional-conformal radiotherapy combining with concurrent chemotherapy in non-small-lung cancer can reached better response and 1-year survival rate,the toxicies was relatively severity ,but it could be tolerated .
