Objective: To observe the expression of Nucle-ar factor of kappa B(NF-κB) and Monocyte Chemoattractant Protein-1 (MCP-1), and the infiltration of mononuclear macrophage CD68 in lupus nephritis(LN). Investigate their relationship between NF-κB , MCP-1 and MΦwith kidney pathology and clinical manifestation.Methods: Renal tissue were assigned to two groups: the lupus nephritis (LN) group and the control group. The 49 renal biopsy specimens were made by renal biopsy from the lupus nephritis patients in Nephrology Department of the second hospital of Shanxi medical university from september, 1997 to september, 2007, who diagnosed as systemic lupus erythematosus (SLE) according to the American College of Rheumatology revised criteria for classification of SLE in 1997.All cases were examined by light microscope, immunofluorescence and transmission electron microscope . These cases were divided into three subtype including 6 LN II,8 LN III and 30 LN IV, 5 LN V according to the classification standard of International Society Nephrology and Renal Pathology Society. LN IV was The most common type,other types were less,therefore, LN were divided into LN IV and no LN IV For 30 and 19 respectively. The 8 cases normal renal were the tissue far from nephronophthisis or tumor of kidney over 2cm.. The expression of NF-κB , MCP-1 and CD68 were detected by immunohistochemistry in nephridial tissue of two groups. The relation between NF-κB,MCP-1 and CD68,and the associativity between they and histological activity index, serum creatinine ,24h urine protein volume were analyzed.Results:(1) Compared with control group , the expression of NF-κB and MCP-1, the infiltration of MΦin LN were higher,especially in typeⅣ,the difference was significance (P<0.01). compared with no LNⅣ, the expression of NF-κB and the infiltration of MΦin LNⅣwere higher, the difference was significance (P<0.01); the expression of MCP-1 were higher too, the difference was significance (P<0.05).(2) The expression of MCP-1 positively correlated with MΦinfiltration in glomeruli and nephric tubul erenal interstitium(r= 0.603和r =0.615,P<0.01). The expression of MCP-1 positive correlated with the expression of NF-κB in glomeruli and nephric tubule erenal interstitium( r=0.524和r =0.526,P<0.01).(3) NF-κB expression in glomeruli and nephric tubule erenal interstitium gromeruli positively correlated with histological activity index,24h urine protein volume and serum creatinine. MCP-1 expression in glomeruli positively correlated with histological activity index and 24h urine protein volume ,no correlated with serum creatinine. MCP-1 expression in nephric tubule correlated with 24h urine protein volume and serum creatinine.no correlated with histological activity index.Conclusion:(1) Macrophage participated in the inflammation of both glomerular and tubulointerstitial areas in LN,its expression was related to the progressive injury of LN.(2) Macrophage infiltration of mesenchymal in glomeruli and nephric tubule may be chemoattracted by MCP-1 .(3) NF-κB induced MΦby activating MCP-1 may be a reason in kidney injury of LN.(4) NF-κB signal pathway may act as a new therapy target to inhibit MΦinfiltration and proliferation in kidney.
