Intracameral Anesthesia In Phacoemulsification Cataract Patients With Diabetes

Objective: To investigate the anterior stability, the effect of mydriasis and the efficacy of anesthesia with the application of intracameral anesthesia in phacoemulsification in the cataract patients with diabetes .Methods: To collect patients of cataract with diabetes from2007\2 to 2008\1 . Patients were randomly divided into 2 groups .The observation group (20cases23eyes) received topical anesthesia with aiercain plud intracameral 1% nopreservative lidocain.The control group (20cases22eyes) received topical anesthesia.All patients underwent phacoemulsification combined with introcular lens implantation with incision on transparent cornea. The efficacy of anesthesia and anterior stability were analysed. Pupil diameters were recorded.Diminution of pupils after emulsification and surgery were calculated.The light reflex postoperative,corneal endothelial cell loss in 1 month postoperative and the surgical complications were observed.Results: The efficacy of anesthesia and diminution of pupils after emulsification and surgery was significantly different between the two groups(P<0.05).The anterior stability in the observation group during the operation is better than that in control group, especially when emulsification top entered anterior chamber ,filling initiated and IOP steped up.Average emulsify time was 48.3 s in observation group,while the average time was 57.6 s in the control group.There was no significant difference between two groups′subjective in the light reflex postoperative,corneal endothelial cell loss and surgical complications,such as postoperative corneal edema, inflammatory reaction,anterior chamber exudation,et al. Three samples in the control group had iris depigmenting.Complications such as hyphema,margo pupillaris bitten, posterior capsular fracture and prolapsus vitrei did not happen in all patients.There was not any serious complications such as cornea decompensation, shallow anterior chamber,retinal detachment in all cases.No adverse reaction of system happened to observation group. Conclusions: Intracameral anesthesia has the effect of mydriasis and stabilizing pupils size and increasing anaesthesia result in phacoemulsificationcataract patients with diabetes.It can lessen the movement of eyeball and the difficulty of emulsification .It decreases the emulsification time and energy damage.For it has no obvious damage to corneal endothelium. It is a safe, effective and feasible method.
