Objectives:By ways of examining the alternation of the bone together with its mucosa in the sinus floor, the biological changer of integration among the top of implants and circumferential tissue in dogs underwent maxillary sinus were perforated in different heighs, to provide experimental evidences in clinical application.Methods:4 healthy adult mongrel dogs, healthy,12-18 monthes old. The weight is 10-15 kg. The first molars of 4 dog's maxillar were extracted after anesthesia and made the immediate implant.The heights of perforation were 1mm,2mm and 3mm,and puncherless to contrast. In the operation we examined that no evident maxillary sinusitis. After 5 monthes 4 dogs were sacrificed and take out of the maxilla with implants. The samples were evaluated and analyzed by general observation,X-ray radiographic observation,histological examination and hard tissue slice observation, Evaluate the integration between the implants and bone..Results:1.All experimental animals were no postoperative inflammation and no sudden death,and all implants were no clinical mobility 2.The X rays showed the excellent interface between the top of implants and maxilla.3. The samples that perforated 1mm and 2mm,maxillary sinus mucosa were no difference between the normal tissue. We did not find that the top of implants perforation.We can saw the top of implants from the maxillary sinus when the heights of perforation is 3 mm.4.Peelled of the maxillary sinus mucosa,top of the implants which perforated 1mm and 2mm were covered with a layer of cortex bone But,the top of implants which were perforated 3mm were no bone covered,but there are bone exists on the implants with maxilla.5 . Through histology observation , HE stain of maxillary sinus mucosa showd stratified pavement epithelium around the implants. Hard tissue slice obserbation showed the integration between the top of implants and bone were firm.Conclusions:1.There was no hurmful effects on the maxillary bone integration that perforation of the maxillary sinus mucosa,and there was no obviously maxillary sinusitis.2.Experimental group which maxillary sinus mucosa were perforated less than 2mm had self-cure and coverd the top of implants intact; newly formed bone coverd the top of implants.3.Experimental group which maxillary sinus mucosa were perforated more than 3mm, the top of implants wre no newly formed bone coverd. But , there are new bone exists on the implants with maxilla. Stratified pavement epithelium around the implants is similar to"cuffs".4 . Immediate implant in posterior maxillary aera,if we need the integration between the implants and bone was firm, we must undertake bone graft, tectorial membrane etc..
