Human papillomavirusvirus(HPV) are a highly diverse family of nonenveloped DNA viruses.Numerous studies have demonstrated HPV DNA in more than 95% of cervical neoplasia and cancer. The annual incidence of HPV infection in the Unite States approaches 6.2 million ,with an estimate prevalence of 20 million.The largest peak in prevalence is in women younger than 25 years ,with infection rates that have been reported to range from 28% to 46%. Most infections are transient and charactered by spontaneous viral clearance but a small proportion may progress to cervical intraepithelial neoplasia (CIN),and eventually to invasive cervical cancer(ICC).About 40% of CIN2/3 persist or progress to ICC.These lesions are the necessary precursor of ICC and have been considered by WHO as the most relevant clinical endpoint for vaccine efficacy studies.Genotype and viral persistence of high-risk(HR)HPV have been shown to be predictors for development of CIN 2/3 or ICC.Objective: To investigate the human papillomavirus(HPV) types detected in outpatient women of LiaoNing ,and to analyze the oncogenesis of the various HPV types.Methods: A total 834 outpatient women were check for 21 HPV DNA types by DNA flow-though hybridization genotyping technique, 90 women who undergo colposcopy and cervical biopsy were collected.According to the pathological diagnosis,the patients were divided into there groups:33 in CIN1 group,27 CIN2 and 30 CIN3.The association between HPV types and different cervical lesions was analyzed. Results:1.There were distribution of HPV genotypes of outpatient in LiaoNingAll of 21 types HPV were detected ,the infection rates of outpation woman 40.4% (337/834),common types(in descending order of frequency) HPV16,6,58,52,11. the infection rates of others types were from 3.7to 0.2%.High-risk types are most frequently found in outpations, the infection rates were 32.6%( 272/834), low-risk were 7.7%( 65/834), multipie infection were 13.3%(111/834).2.There were distribution of HPV genotypes in different age-group.According to ages,the patients were divided into four groups,HPV genotypes(in descending order of frequency)63.1% , 40.0% , 31.4% ,30.7%.The infection rates of HPV were not correlated in ages( P>0.05),the peak in prevalence were in women young than 30 years.3. There were distribution of HPV genotypes in CIN .(1)A total of 90 CIN histological specimens collected between 2006 and 2007 were included in the analysis. Median age was 38 years (range23-59). The infection rates of patient with CIN were 74.4%(67/90), HPV16 was by far the most prevalent type accounting for 74.4%(67/90)of all specimens ,followed by HPV 58(16.6%,15/90),HPV33(6.6%,6/90)and HPV31(3.3%,3/90)。(2)The infection rates of patient with CIN1,CIN2,CIN3 were 54.5%(18/33),74.1%(20/27),96.6%(29/30)。(3)Ninety-eight percent(88/90)of histological specimens had a HR infection,two further cases were infected by a LR type only (HPV6 and HPV42).4.HPV16 were by far the most common in patients with CIN,followed HPV58.With HPV16 and /or HPV58 prevalence of 60% in CIN and 65% in CIN3。Conclusion:1.HPV types 16,6,58,52 and 11 are more common in outpatients at Liaoning, heterogeneity in HPV type distribution among women from different reglons.2.HPV-DNA is present in 98% 0f CIN cases suggesting that almost no high-grade lesion develop independently of any HPV infection. 3.HPV types 16,58,33, and 31 are more common in patients with CIN. HPV16 is by far the most common HPV type associated with CIN. HPV16 and/or HPV58 were associated with CIN at Liaoning.
