Study Of The Relationship Between Heart Rate Turbulence And Severity Of Coronary Lesion In Patients With Coronary Heart Disease

Objective To investigate relationship between heart rate turbulence(HRT)and severity of coronary lesion in patients with coronary heart disease(CHD),and compare with one of heart rate variability SDNN.Method We studied 116 suspected patients of angina pectoris.All the patients were received monitor of 24 hour Holter.turbulance onset(TO)and turbulance slope(TS)of heart rate turbulence was obtained,as well as SDNN. All the patients were received coronary angiography(CAG).patients were divided into CAG(-)group and CAG(+)group according to the results of CAG,patients in CAG(+)group were divided into single-vessel,double-vessel and triple-vessel coronary lesion.Severity of coronary lesion was evaluated by numbers of lesion coronary artery and Gensini's coronary angiography score. Finally,we analyzed relationship between HRT and severity of coronary lesion in patients with CHD,as well as relationship between SDNN and severity of CHD.Result TO in CAG(+)group was significantly higher than in CAG(-)group (P＜0.001),while TS and SDNN was lower(P＜0.001).TO in triple-vessel lesion group was higher than in single-vessellesion(P＜0.05)and double-vesel lesion group(P＜0.05);while SDNN was lower(P＜0.05);TO was higher in double-vessel lesion group than single-vessel group,while SDNN is lower,both no statistically significance were observed(P＞0.05).TS,which was lower in double-vessel lesion group than in single-vessel lesion(P＜0.05),was lower in triple-vessel lesion group than in single-vessel lesion group(P＜0.05)and double-vessel group(P＜0.05).In correlation analysis between TO,TS,SDNN and Gensini's coronary angiography score.,an inverse linear trend between TS and Gensini's coronary angiography score was highest,(r=-0.719,P＜0.01). decrease of TS is asscociated with significantly increase Gensini's coronary angiography score.TO was the second highest,(r=0.546,P＜0.01),SDNN was the lowest,(r=-0.481,P＜0.01).In correlation analysis between numbers of lesion coronary artery and TO,TS,SDNN.TS was highest correlation, (rs=-0.637,P＜0.01),the more numbers of guilty coronary artery,the lower variables of TS.Conclusions Heart rate turbulence is significant correlation with severity of coronary lesion in patients with CHD.Espeally,relationship between TS and Gensini's coronary angiography score was highest,decrease of TS is asscociated with significant increase of coronary lesion.HRT is an effective parameters to predict severity of coronary lesions in patients with CHD and find high risk patients of CHD.
