【Objective】To study the effect of mild hypothermia therapy(34℃~35℃,kept 7 days)on immunologic function and hospital-acquired pneumonia in patients with severe traumatic brain injury and to evaluate the safety of mild hypothermia therapy under certain period of time and temperature..【Methods】60 patients were randomly divided into hypothermia therapy(HT) group(n=30)and non-hypothermia therapy(NT)group(n=30).In the HT group,the rectal temperature(RT)was controlled at 34℃~35℃,kept 7 days and 37.5℃~38.5℃in the NT group.The other therapies were the same.In each group,the dynamic change of immunoglobulin G,A and M(IgG,IgA,IgM)and the serum lymphocyte transformation rate(LTR)were detected on the first,third and seventh day. Monitoring hospital-acquired pneumonia and spectrum of bacterium after 48 hours~7days in two groups,the ventilator associated pneumonia was also observed.【Results】The level of the serum LTR and IgG,IgA,and IgM were not significantly different at the first,third and seventh day in two groups.The incidence rate of hospital-acquired pneumonia in HT was higher than that in NT.One is 33.3%,the other was 26.7%,but had no significant difference in statistics.The incidence rate of ventilator associated pneumonia in HT was 38.1%while the other is 29.4%.Also,it had no significant difference in statistics.The difference of the structure of bacterium in hospital acquired pneumonia was not found in two groups.【Conclusion】The 34℃~35℃mild hypothermia therapy cannot inhibit the immunologic function of severe brain injured patients and increase the incidence rate of ventilator associated pneumonia.Slightly increased hospital acquired pneumonia has no relationship with the immunologic function of patients,but the long run imply of the ventilator.According to this research,short term mild hypothermia therapy is safety,feasible.
