Study For The Relationship Between The Level Of Lymphocyte Subsets With The Theory Of Wei Qi Ying Xue

Purpose:By sepsis pathological process,sepsis,severe sepsis and Septic shock, the patients of various were pointed according to Chinese medicine wei qi ying xue,Do the acute physiology and chronic health evaluationⅡand determination of the different time in patients with Subgroups lymphocytes(CD3+,CD4+,CD8+)in the blood content,And make a correlation analysis,Aimed at understanding the qi fen,ying fen,xue fen sepsis patients critical condition and the degree of immune suppression characteristics of the state,To further clarify the pathogenesis of sepsis,the combination of Western medicine in the treatment of immune lay the foundation for intervention.Methods:Will be in April 2007-2008,in our hospital ICU,respiratory and general surgery collected with the diagnosis of sepsis 30 patients,into branches with 24 hours by weiqiyingxue differentiation,into a qifen,yingfen,xuefen group the ten cases,located ten cases as a normal control group,while 30 patients for the APACHEⅡscore,A sample of patients and normal peripheral blood injected 1ml,Application of FCM detecting the content of lymphocyte subsets, for the analysis.Results:1.The sepsis patients with APACHEⅡscores:the xuefen grouping score higher than the qifen and yingfen(p＜0.01),the yingfen grouping score higher than qifen(p＜0.01).Application Spearman collection analysis.The theory of weiqiyingxue and the APACHEⅡwas positively corrected(p＜0.01).2.The control group of patients with sepsis and permit lymphocyte subsets in comparison:Comparing the level of CD3+:the qifen group is lower than the control group(p＜0.05);the yingfen group is lower than the control group(p＜0.01); the xuefen group is lower than the control group(p＜0.01);the yingfen group is lower than the qifen group(p＜0.05);the xuefen group is lower than the yingfen group(p＜0.05);the yingfen group and xuefen group was no significant difference(p＞0.05).Comparing the level of CD4+:the qifen group is lower than the control group(p＜0.05);the yingfen group is lower than the control group(p＜0.01); the xuefen group is lower than the control group(p＜0.01);the yingfen group is lower than the qifen group(p＜0.05);the xuefen group is lower than the yingfen group(p＜0.05);the yingfen group and xuefen group was no significant difference(p＞0.05).Comparing the level of CD8+:the qifen group is lowerer than the control group(p＜0.05);the yingfen group is lowerer than the control group(p＜0.05); the xuefen group is lowerer than the control group(p＜0.01);the qifen group and yingfen group was no significant difference(p＞0.05);the qifen group and xuefen group was no significant difference(p＞0.05);the xuefen group is lowerer than the yingfen group(p＜0.05).According to the above results,TCM show type,and from qifen→yingfen→xuefen,CD3+,CD4+,CD8+ levels decreased gradually,Application Spearman collection analysis:the differentiation period of weiqiyingxue and CD3+,CD4+,CD8+ Lymphocyte was negatively corrected(p＜0.01).3.APACHEⅡscore and CD4+/CD8+ correction analysis:Application pearson correction analysis:CD4+/CD8+ and APACHEⅡwas negatively corrected(p＜0.01).4.28-day mortality compared:the qifen group 10%(1/10)with the yingfen group 42%(5/10),no significant difference(p＞0.05);with the xufen group 60%(9/15),there was significant difference(p＜0.05);the yingfen group with xuefen group,no significant difference(p＞0.05),the xuefen group showed a higher mortality rate than qifen group.5.death group and the survival group CD4+ Lymphocyte and the APACHEⅡscore comparison:Death group below the level of CD4+ lymphocyte survival group(p＜0.01).died of APACHEⅡscore higher than those who survived(p＜0.05).Conclusions:1.Sepsis is a Chinese "wen disease" "fever" areas,Sepsis pathological process of sepsis,severe sepsis,septic shock and the weiqiyingxue differentiation from qifen→yingfen→xuefen basically the same process.Note in clinical work,will be doing the theory of weiqiyingxue used Syndrome sepsis.2.Lymphocyte subsets involved in the pathogenesis of sepsis,And in the pathogenesis of sepsis in different stages,the more serious condition,CD4 + in the lower the mortality rate higher.CD4+/CD8+ ratio,the lower the higher mortality rate.Note lymphocyte subsets of a better response to the evolution of pathology sepsis,sepsis can be used as a clinical condition and prognosis of indicators.3.Correlation analysis showed that the theory of weiqiyingxue and the APACHEⅡscore was positively correlated,the theory of weiqiyingxue and CD3+,CD4+,CD8+lymphocytes was negatively correlated.The qifen patients with sepsis,APACHEⅡscore low,CD4+/CD8+ level slightly higher,the mortality rate low.yingfen and xuefen patients,APACHEⅡscore relatively high, relatively low level of CD4+/CD8+,the high mortality rate.Clinical will combine the three,will contribute to more accurate judgment of the severity and prognosis of sepsis.
