Object:Analyze clinical and radiographic results following cervical arthroplasty of patients with cervical spondylosis.Evaluate its long term clinical efficacy and guide clinical treatment.Materials and Methods:We reviewed 44 patients with cervical arthroplasty between 2005 and 2007.All patients were diagnosed as cervical spondylosis preoperatively and 11 patients with single cervical arthroplasty were followed for at least 24 months.All patients were evaluated by clinical efficacy and radiographs preoperatively and 13 12 24 month postoperatively.Clinical efficacy were analyzed by JOA NDI VAS and Odsm's criteria.Static and dynamic radiographs were made to measure ROM of treated segment,adjacent segments,the FSU and sagittal alignment of the FSU and the cervical spine to ascertain its effects on biomechanics of the cervical spine.CT and MRI were also made at 12 and 24 months postoperatively.Periods were categorized as early follow up(<6 months)and late follow up(>6 months).Results:JOA,VAS and NDI were all improved postoperatively and the differences had statistical significances.ROM of treated segment,adjacent segments and the FSU all decreased at early follow up,but they all returned to normal level and the differences had no statistical significance.The lordotic curvature of the FSU and the cervical spine all increased postoperatively with time going and the difference had statistical significance.No prosthesis related complications were found.No spinal cords and nerve roots were recompressed again and no adjacent disc obvious degeneration were seen from CT and MRI.Also no heterotopic ossification were founded during our follows.Concusions:Patients with cervical arthroplaty have a good clinical effect and ROMs of the treated segment,adjacent segments,the FSU and the cervical spine are all maintained postoperatively.The curvature of the FSU and the cervical spine are reestablished.It can prevent accelerated degeneration of adjacent segments.But a longer follow is still needed.
