Objective: To investigate the affection of approach and cage on the low lumbar interbody fusion rate.Methods: A retrospective study was performed , 96 surgical cases of lower lumbar interbody fusion were collected from the traumatic orthopaedics department of Qilu hospital from Feb, 2004 to May, 2008. There were 52 cases of spondylolisthesis, including 26 males and 26 females; 18 cases of degenerative low lumbar spinal instability, including 10 males and 8 females; 16 cases of protrusion of the lumbar interverteral disc, including 8 males and 8 females; 10 cases of stenos is of lower lumbar, including 6 males and 4 females. Their ages were from 35 to 78 and average age was 52.23.There were four approach been adopted, including ALIF, PLIF, TLIF and ILIF. All cases were undergone with internal fixator. They were divided into two groups randomly when dealed with interbody.Only bone-graft group:one tricortical bone grafts was conducted .Cage group: two quadrate PEEK cages filled with bone were conducted parallely. Following markers were observed at the 3rd month, 6th month , 12th month and 24th month postoperatively.Results : All cases obtained preferably, and the fusion rate of four approachs had no difference on statistics after 24 months(t=20.537, P=0.593).But,there was significant difference between two groups , the Only bone-graft group excelled the Cage group on statistics (t=22.617, P<0.005).Conclusion: There was significant relativity between the fusion rate and cage,but not to approach. The philosophy on how to chose certain kind of surgical approach was based on their own understanding of the pathologyof lumbar and fitness. It's very useful to the fusion rate in adopting internal fixator.If possible,we should graft more bone in interbody.
