One Case Report Of Budd-Chiari Syndrome With Radical Correction Therapy And Literature Review

Objective:By analyzing 1 case of Budd—Chiari Syndrome(inferior vena cava type)treating by radical correction,to recommend the operation principle of radical correction and to approach the indications and contraindications.To appraise the clinic effect and application prospect about the radical correction after observing the effect in the near future and long-term.Clinic information:The patient,male,42-year-old,worker,managed in hospital because of gingiva bleeding for 3 years and the lower limbs edema with emaciation for 6 months on Mar 1^st,2005.The result of inferior vena cava opacification is that inferior vena cava near right atrium was being serious stenosis and mass thrombosis form in the inferior vena cava.The diagnosis was Budd—Chiari Syndrome(inferior vena cava type).using the cardiopulmonary bypass unit to transfuse the blood,radical correction completely ectomized the septum and recovered the normal structure.Results:1 The effect of radical correction was satisfactory(all the examinations in charts 1and 2), and gingiva bleeding lightened by degrees,and the lower limbs edema with emaciation obviously lightened.the color of urine became normal.When it was 50 days after operation,the patient feel to breath difficultly,the X ratios exam showed pleural effusion in the right thoracic cavity.Treating by medical treatment for only one week,pleural effusion became less,and the symptoms disappeared.When it was 2years and 9months after operation,the patient appeared melena once,and hematemesis.The total dose of blood rejected was more than 1000 ml.So the patient came back and treated by percutaneoustransluminal angioplasty(PTA),hematemesis and melena dis appeared and the patient discharged.Conclusion:the radical correction using the cardiopulmonary bypass unit to transfuse the blood can reduce the risk of the surgical treatment and its manipulation is easier than before.this method don't need many complicated instrument,and this make it easier that launching it in the base hospital.Because it recovers the normal structure of inferior vena cava,and the effect in the near future and long-term is more satisfied than interventional therapy,this method has excellent prospect.
