Objective:(1)Detected the expression of IDO and IL-10 value in placenta of normal pregnancies and patients with pre-eclampsia to evaluate the function of IDO and IL-10 in the pathogenesis. (2)To evaluate the correlation between the level of IDO and IL-10 in placenta.Methods : 1 The study subject:(1)The pre-elampsia group:20 patients with pre-elampsia who had prenatal examinations and delivered in the Obstetrics of the Second Hospital of HeBei Medical University were selected from January 2007 to May 2007.The study group including 10 patients with early onset pre-eclampsia and 10 patients with late onset pre-elampsia .Mean age of early onset pre-eclampsia group was 28.8±4.03 years old (24 to 35 years old),and mean gestational age was 33.54±1.24 weeks (31+2 to 35+1weeks of gestation). Mean age of late onset pre-eclampsia group was 28.50±4.55 years old (22 to 31 years old), and mean gestational age was 37.37±1.98 weeks (35 to 41+1weeks of gestation).They were diagnosed according to the diagnostic standard(Obstetrics and Gynecology,the Sixth Edition).(2) The control group: 15 healthy pregnant women, who were deliveried in our hospital in the same time. Mean age of them was 27.80±2.94 years old(24 to 33 years old), and mean gestational age was 39. 89±0.66 weeks(38+6 to 40+4weeks of gestation). All the subjects must meet the following criteria: primiparae,single pregnancy ,no history of recurrent abortion, no assistance of reproduction technique,no complications of intered medicine and other complications of pregnancy . All the pregnancy termination of both groups was cesarean section.2 Sample collecting:Each specimen(1cm×1cm×1cm)was collected from central placenta after cesarean section and was blotted blood stain by dry gauze.Then they were fixed in 10% formalin and 24h-48h later embedded in paraffin.3 The experiment menthods:Samples stained by HE and then detected the expression of IDO and IL-10 value by immunohistochemistry.All samples were processed under the same conditions with positive control and negative control(PBS).An average of 4 fields caesarean surgery before uterine contraction. OD was used to assess the immunostaining intensity of trophoblast cells. concentration of culture human cytotrophoblast cells. Detect the location of IDO in the placenta under laser scanning confocal microscopy (LSCM).4 Statistics analysis:All the data were processed by SPSS13.0.All values were expressed as mean±standard(x_±s) deviation and differences were compared using Student's t-test and one-way ANOVA.Pearson correlate was made for correlate analysis.α<0.05 was considered statistically significant.Results:1 IDO were mainly located in the cytoplasm of placental villus syncytiotrophoblast and the nucleus and the cytoplasm of invading extravillous cytotrophoblas(tEVT)in the Nitabuch's layer,less in the cytoplasm of placental villus vascular endothelial cells and lymph-cell. IL-10 were mainly located in placental villus syncytiotrophoblast and invading EVT in the Nitabuch's layer.2 The expression of IDO and IL-10 on syncytiotrophoblast of normal villi was stronger than pre-eclampsia placenta (OD: 0.1600±0.0150, 0.1567±0.0187 vs 0.1359±0.0189, 0.1290±0.0331), The differences were significant in statistics(P<0.05).3 The expression of IDO and IL-10 on normal invading EVT in the Nitabuch's layer was stronger than pre-eclampsia placenta(OD: 0.1656±0.0239, 0.1561±0.022 vs 0.1371±0.0262, 0.1277±0.0315),The differences were significant in statistics (P<0.05).4 The expression of IDO and IL-10 on syncytiotrophoblast of late onset pre-eclampsia villi was stronger than early onset pre-eclampsia placenta(OD: 0.1417±0.0211, 0.1278±0.0421 vs 0.1294±0.0147,0.1302±0.0232),there was no statistical differences(P>0.05).5 The expression of IDO and IL-10 on invading EVT of late onset pre-eclampsia was stronger than early onset pre-eclampsia placenta(OD: 0.1444±0.0279, 0.1288±0.0378 vs0.1288±0.0230, 0.1278±0.0248),there was no statistical differences(P>0.05).6 The changes about the expression of IDO and IL-10 in normal group and pre-eclampsia group were positive correlation with Birth weight and negative correlation with mean arterial pressure(MAP). The relation between the level of IDO and IL-10 on invading EVT of normal pregnancy group and pre-eclampsia group was positive correlation (r=0.475,P<0.05) .Conclusion:1 IDO was mainly located in both the syncytiotrophoblast of placental villus and EVT in the Nitabuch's layer.The level of IDO in placenta of pre-elampsia group was significantly lower than that of normal pregnancy group,suggesting IDO may participate in pathogenic process of pre-eclampsia.2 IL-10 was mainly located in both the syncytiotrophoblast of placental villus and EVT in the Nitabuch's layer.The level of IDO in placenta of pre-elampsia group was significantly lower than that of normal pregnancy group,suggesting IL-10 may participate in pathogenic process of pre-eclampsia.3 Since EVT are the fetal cells with the closest contact to the maternal immune system, our results suggest that it is EVT which express IDO and IL-10 protects the fetus from rejection by downregulating local maternal T-cell responses.4 In the research result,the level of IDO and IL-10 in placenta of pre-eclampsia group was significantly lower than that of normal pregnancy group,and Pearson correlation analysis result:the relation between the level of IDO and IL-10 on invading EVT of normal pregnancy group and pre-eclampsia group was positive correlation,suggesting the reduced level of IDO in placenta from pre-eclamptic pregnancies was not due to direct inhibition of IDO production by IL-10. The level of IDO regulated by IL-10 may through many pathes.
