Study on the Immunoarchitecture of Skin Epithelium and the Express ion of Cytokeration in Cutaneous TumorObjectiveHistology classification of skin epithelial tumor including scarfskin and skin appendages,depending on direction of histodifferentiation from tuomr to normal skin.It is hard to be diagnosised according to complex tumor structure.Sometimes the differential diagnosis is difficult.It plays an important role in studying cutaneous tumor differentiation structure,explaining morphology characteristics morphology and disceming innocent from malignant cutaneous tumor through study the structure, the morphous and function of the skin tissue.It is a new view angle of skin histologic and cutaneous tumor diagnosis to observe skin immunstructure and reveal various kinds of histologies by immunohistochemistry marker.Cytokeratin is a specific marker for epithelial cells,cytokeratin's expression differs from different kinds of epithelial cells and differentiation stages of the same kind.This experiment try to describe skin epithelium immunstructure and explore practical application valve of cytokeratin on epithelial tumor diagnosis and differential diagnosis by studying cytokeratin expression feature in normal skin and different epithelial tumors.Method66 cases of cutaneous tumor including 12 cases of squamous cell carcinoma,15 cases of basal cell carcinoma and 39 cases of skin appendages tumor were provided by dermatology department and pathology department of Shengli Oil Field Central Hospital.All cases had final diagnosis by clinical symptoms and pathologic diagnosis.39 cases of skin appendages tumor include 9 cases of pilomatricoma,3 cases of external tricholemmoma,2 cases of trichofolliculoma, 1 cases of trichblastoma,4 cases of external root sheath carcinoma,5 cases of corpora mammillaria eccrine spiradenoma,4 cases of clear cell hidradenoma,3 cases of eccrine spiradenoma,2 cases of springocystadenoma papilliferum,1 cases of syringocystadenoma papilliferum,3 cases of chondroid syringoma(mixed tumor of skin),2 cases of sebaceous adenocarcinoma.20 cases of normal skin samples as control group including chest skin away from tumor in resectin samples of 5 cases breast cancer radical operation,anus skin away from tumor in resectin samples of 5 cases rectal cancer radical operation and cutting edge skin from 10 cases of pigmented mole resectin sample.Cytokeratin's expression in different cells of different epithelial structure is observed by tagging CK7(K72.2),CK8(C-51),CK10(DE-K10),CK14(LL002),CK17(E3),CK18(DC10),CK19(KS19.1) with immunohistochemistry S-P method.All immunochemistry staining outcome was observed and judged undern light microscope.Result was semiquantitative decided by percentage of positive cells and positive cells staining intensity refer to method of Xu Liang-zhong.The numeration data statistical treatment applied withχ~2 analysis and count data with t analysis.ResultCytokeratin expresses in epiderm and skin appendant organ which includes hair,sebaceous follicle,coil gland,apocrine sweat gland.This experiment displays that changes of cytokeratin's expression from low molecular mass to high molecular mass while gradually differentiation courses of epithelium,including hair follicle and glandulae sebaceae.Cytokeratin's expression in sweat gland is different from glandulae sebaceae,epithelial acanthocyte and granulation layer cells.Cytokeratin's expression is different in different kinds of cutaneous tumor.CK10 was expressed in squamous cell carcinoma but not expressed in basalioma;CK19 expression is found in basalioma but not found in squamous cell carcinoma. Cytokeratin's expression and distribution is different in different kinds of cells in skin appendages tumor.Cytokeratin expression is widespread in most tumor differentiated from hair.Different kinds of cytokeratin expression places is different in tumor differentiated from sweat gland.CK10 and CK14 expresses in highly differentiated glandulae sebaceae secretory cells,CK17 expresses in glandulae sebaceae tube cells and scalelike cells differentiated from tumor cells.The expression of CK8,CK10,CK14,CK18 and CK19 is different between squamous cell carcinoma and basalioma according to statistical treatment.The expression of CK7,CK8,,CK17,CK18 and CK19 is different between hair tumor and sweat gland tumor.Conclusion1.Skin epithelial component contains some immunstructure.Immunstructure of epidermal cell is identified,expression table of skin appendages cells is comparably wide,overlap is frequent.2.CK10 and CK18 expression is high in squamous cell carcinoma,CK8,CK14,CK19 expression is high in basalioma.3.CK17 expression is high in hair tumor,CK7,CK8,CK18 and CK19 expression is high in sweat gland tumor.4.Positive cells distribution should be noticed as CK expression is different in different places of cutaneous tumor.5.It can help distinguish skin epithelial tumor to detect selectively combined expression of one platoon of CK.6.The study of CK expression in skin and tumors may be important for research on cutaneous tumor stem cells.
