Objective:To assess the safety and efficacy of surgery which accomplished corpectomy,interbody fusion with allograft bone and posterior instrumentation via single posterolateral approach in the thoracolumbar metastasis,in which tumor extending into the spinal canal, the bilater pedicles or the paravertebral areas.Methods:Between Jan 2007 and Jan 2008,a total of 23 patients with thoracolumbar metastasis were admitted,according to the indications:1)obvious pains;2)neurological deficits;3)Tomita surgical staging:type 4-6,Tomita scoring system:4-7 points;4)good general condition and predicted survival>3 months,10 of which were performed corpectomy,circumferential spinal cord decompression and spinal reconstruction with interbody fusion with allograft bone and pedicle fixation via single posterolateral approach.The outcome was assessed by the improvement in pain relief,neurological symptoms and interbody fusion.Results:Obvious pain relief in all patients immediately after surgery; the neurological function have improved at least one grade in ASIA and bone fusion have reached at least gradeⅡat the last follow up(≥6 months).None was dead in operations or suffered infections and failed instrumentation.Conclusion:For the patients with thoracolumbar metastasis,which tumor extending into the spinal canal,the bilater pedicles or the paravertebral areas,the surgery is a safe and valid surgical choice,which accomplished corpectomy,interbody fusion with allograft bone and posterior instrumentation via single posterolateral approach.
