ObjectiveTo study the characteristics of 64-slice CT perfusion parameters and relations perfusion parameters of hepatic carcinoma with serum tumor marker(alpha-fetoprotein,AFP);Study of metastatic tumor of liver with 64-slice CT perfusion parameters and its clinical value.Methods1,20 cases of normal control group and 40 patients of hepatic carcinoma have been performed CT perfusion scanning,then the values of flow perfusion(BF),blood volum(BV),time to start(TTS),time to peak(TIP),patlak blood volume(pBV),permeability(P),arterial liver perfusion(ALP),portal venous perfusion(PVP)and hepatic perfusion index(HPI)were calculated respectively.2,The 23 cases(40 lesions)in patients liver metastases with CT perfusion scanning,to access blood flow perfusion(BF),patlak blood capacity(pBV),vascular permeability(P, permeability),hepatic arterial perfusion(ALP),portal venous perfusion(PVP),hepatic arterial perfusion index(HPI)parameters.Results1,In primary hepatic carcinoma group,the value of BF,HPI,ALP in center lesions were significantly higher than the edge,but TTS,TTP were shorter than the edge,and P was slightly lower than the edge.2,The perfusion parameters of the normal control respectively were BF48.65±9.41 ml/100ml/min,BV 136.58±18.23(1000:1),TTS 2.51±0.31s,TTP 18.63±3.88s,P 69.12±12.21(0.5ml/100ml/min),ALP25.83±2.68ml/100ml/min,PVP103.85±15.44ml/100ml/ min,HPI 22.11±4.01%;The values of BF,BV,TTS,TTP,P,ALP,PVP,HPI in primary liver carcinoma group were 92.68±13.29ml/100ml/min,132.10±31.59(ml/L),1.63±0.25s,12.49±2.73s,38.65±12.71(0.5ml/100ml/min),297.39±35.31 ml/100ml/min,99.24± 10.18 ml/100ml/min,78.14±13.31%respectively.3,In AFP positive group,AFP has relations with pBV,ALP and HPI of primary liver cancer.4,In liver metastases group,the normal liver tissue perfusion parameters BF,pBV,P,ALP, PVP,HPI were 47.85±18.11 ml/100ml/min,41.25±6.32(ml/L),64.25±11.10 (0.5ml/100ml/min),24.55±6.96 ml/100ml/min,105.24±16.42ml/100ml/min,20.18±3.81%respectively.5,In liver metastases group,the normal liver tissue and the lesions center compare,BF,pBV,P, ALP,HPI have statistics significance(P<0.01),PVP(P<0.05);The normal liver tissue and the stove week organization compare,pBV,ALP,HPI have statistics significance(P<0.01), but BF,P,PVP non-statistics significance;The center and edge of lesions compare BF,P to have statistics significance(P<0.01)pBV,ALP(P<0.05);CT perfusion images clearly show the tumor borders,the perfusion images size of lesions detected are larger than original images.ConclusionsCT perfusion imaging have good values,such as the early diagnosis of hepatic carcinoma and liver metastatic tumor diagnosis and helps determine the clinical surgical resection,and can reflect the changes in hepatic carcinoma lesions.
