Objective:To study the expression pattern of survivin and caspase-3 between a large part and the less part of dissymmetric BPH. Then clarify the role of apoptosis in BPH mechanism.Methods:BPH tissues from 25 patients who have TURP were embedded in paraffin, which cystourethroscope show those prostates were severely dissymmetric. The samples are from the two parts of each person. One part is from large hyperplasia gland and the other is from less hyperplasia counterpart. 50 paraffin blocks were cut into sections and stained by survivin and caspase-3 of polyclonal antibody against human. Paired samples test analyses difference between two groups expression of survivin and caspase-3. Analyses were performed using the Statistical Package for Social sciences software program (SPSS 13.0 for Windows). Differences with P-value less than 0.05 were considered significant.Results:1. The survivin immunoreactivity was localized to the nuclei of prostatic cells. Survivin expression in large hyperplasia part (22.08%±16.33%) was significantly higher than the less part(7.56%±9.18%) in dissymmetric BPH (P<0.01).2. Positive nuclear or cytoplasm staining was also seen in the caspase-3 immunoreactivity. Caspase-3 expression was higher in less hyperplasia part(51.52%±27.27%) than in large hyperplasia part(33.12%±21.01%) in dissymmetric BPH (P<0.01).3. Survivin expression in large hyperplasia part of BPH was inversely correlated with caspase-3 (r = -0.474, P< 0.05).but in less hyperplasia part of BPH, the expression of the two factors has no relation(r = -0.313, P>0.05).Conclusions:Survivin and caspase-3 express distinguished between the two part of dissymmetric BPH ,which indicates that apoptosis plays an important role in BPH development. In large hyperplasia part of BPH, survivin inhibits the caspase-3 effect in apoptoic process, which leads to disorder of cell growth.
