Objective:To explore the key factors which influence the long-term prognosis of Henoch-schonlein purpura and risk factors involving kidneys through retrospective analysis of 241 children with HSP.Methods:241 children are chosen to our subjects,with a diagonosis of HSP in Shang Xi Medical University First Hospital from January 2004 to August 2007.The inclusion criteria for HSP refers to ZhuFuTang practice Peadiatrics 7 edition,excluding the patients who had nonthtombocytopenic purpura or bad compliance.The clinical informations about the children such as sax,age,onset season,inducing factors,laboratory findings and et.al are all obtained through clinic records or by telephone.Our subject are divideded into two groups(non-nephritis group A,nephritis group B) by the extent of kidney involvement.Data were analyzed for statistical significance by means of X~2 test,ANOVA and binary Logistic regression.Result:Epidemiology:Our population are consisted of 146 boys and 95 girls ranging in age from 0.5 to 14 years,The mean age was 9±2.7 years.Patient may present in every month of the year,with peak in spring months.28%children has evident induced factors,such as infection,food and others,among which the upper respiratory infection and sea food separately occupy 20%or 6.6%ratio.Clinical feature:The major clinical feature of HSP are purpura,arthritis,abdominal pain,gastrointestinal bleeding(occult bleeding,gross bleeding), nephritis(Nephritis was defined as the presence of gross or microscopic hematuria(>5 red blood cells per high-power microscopic field in a centrifuged specimen) with or without proteinuria.), especially nephritis ofen has longer duration of symptoms than others,which heavily influences the long term prognosis of the illness.As to the risk factors,we make binary Logistic regression analysize about sax,age,peripheral blood platet count and white cell count,the level of serum IgA and C3,it is indicated that the high peripheral blood platet(>300×10~9/L) and old age(>7) are the two main risk factors,the odd risks are separately 3.33,4.714,meawhile the 95%confident interval exclude 1.Conclusion:HSPN is the key factor which influence the long-term prognosis of HSP.The old age and high platelet count are the main risk factors of HSPN.
