Nasopharyngeal carcinoma is one of the highest occurrence tumor in China,the disease incidence rates is on top of the Otolaryngology malignant tumor.Nasopharyngeal carcinoma is most likely found at the pharynx nasalis's infundibuliform recess,distal end or posterior wall, secondly is at lateral wall, however it is seldom found on front or bottom board. The symptom in early stage of the disease is atypical,it can be the tears with blood,half-Tinnitus,Ear plug feeling or headache.Abundant researches shows that many of the Differential Expression Genes are found in the process of the development of the Nasopharyngeal carcinoma,the related Genes include:Signal and Protein Gene,Cancer Gene and Original Cancer Gene,Immune Related Gene,Demise Gene,DNA integration and transcription factors etc.At present it is considered that the occurrence of Nasopharyngeal carcinoma is related with hereditary factor,virus infection,environment factors and so on. All these factors ultimately embody on the influence on the expression level of gene and glair .In recent years the researches indicate that cyclooxygenase-2 and epidemal growth factor receptor not only participate the cell proliferation and differentiation but also play a role in tumorous metastasis and vascularization. It promotes the happening and development of tumor. However there are not many reports about nasopharyngeal carcinoma and the results are commonly agreed neither. Ki67,a Cycle related gene,is a cell proliferation nucleoantigen.It's expression relates with many kinds of tumorous happening,development.infiltration.staging and metastasis. Some study manifest the index can be used for evaluation of the prognosis of malignant tumor and the judgment of the dangerous level of the remote organ metastasis. p53 an important anti-oncogene is the main negative regulation of cellular proliferation.It sustains cell normal function by regulating cell life cycle and initiating cell apoptosis .Objectives:To investigate the expression of COX-2,EGFR,Ki67 and p53 between tissue of Nasopharyngeal carcinoma and chronicity blennymenitis of pars nasalis pharynges.To analysis the correlation between them. To help diagnosis , treatment ,and assessment the prognosis.Materials and methods:Collected 44 cases paraffin block of NPC between 2002 to 2007 from Dalian Medical university affiliated number 2 clinic hospital, 32 cases with metabasis of lymphonodi cervicales and 12 cases without metabasis of lymphonodi cervicales. In this work,32 cases of paraffin block of pars nasalis pharynges were spontaneous collected at same period for comparison. All of specimen were made common histological section with HE dyeing and observed by light microscope to definite pathologic diagnosis again.Then Immunohistochemical detect the expression of COX-2,EGFR,P53 and Ki67.The statistical analysis of the experimental result was performed by rank sum test and the correlation judgment used, the significance of difference (P<0.05).Results:1 The average positive expression of COX-2 was 3.9886±2.31900; and the positive expression rates of COX-2 in NPC was 93.2%. But those were 5.6250±2.64880 and 100% respectively in chronicity blennymenitis of pars nasalis pharynges.The expression of COX-2 in chronicity blennymenitis of pars nasalis pharynges was higher than that of in NPC (P<0.05).2 The average positive expression of EGFR was 3.0682±2.47215; and the positive expression rate of EGFR in NPC was 79.5%. But those were 5.5156±2.16081 and 100% respectively in chronicity blennymenitis of pars nasalis pharynges. The expression of EGFR in chronicity blennymenitis of pars nasalis pharynges was higher than that of in NPC (P<0.05).3 The average positive expression of Ki67 was 25.7841%±26.70345%; and the positive expression rate of Ki67 in NPC was 72.7 %; and High expression rate was 34.1%. But those were 5.7656%±6.24627% and 37.5% respectively in chronicity blennymenitis of pars nasalis pharynges; and High expression rate was 0%. The expression of Ki67 in NPC was higher than that in chronicity blennymenitis of pars nasalis pharynges(P<0.05).4 The average positive expression of p53 was 25.7841±26.70345;and the positive expression rate of p53 in NPC was 72.7%. But those were 5.7656±6.24627 and 37.5% respectively in chronicity blennymenitis of pars nasalis pharynges . The expression of p53 in NPC was higher than that of in chronicity blennymenitis of pars nasalis pharynges (P<0.05). 5 There is no much difference for the expression of COX-2 ,EGFR, Ki67 and p53 in NPC no matter there is or not lymphonodi metabasis existing (P>0.05).6. The expression of COX-2 and EGFR to be positive correlation in NPC. And the coefficient correlation was 0.349 (P<0.05).Conclusion:1. The positive expression of COX-2 and EGFR in nasopharyngeal carcinoma and in chronicity blennymenitis of pars nasalis pharynges tissue is much higher.That indicates that they can not be used as useful indicators for pathological diagnosis of nasopharyngeal carcinoma.2. The expression of COX-2 and EGFR to be positive correlation. That indicates that the COX-2 and EGFR are cross affected during the process of happening and development of the nasopharyngeal carcinoma. They might impose the process together or corporately.3. The positive expression of Ki67 in nasopharyngeal carcinoma tissue is much higher, but in non-tumour nasopharyngeal tissue is much lower. So Ki67 can be one of the index for assessing malignancy degree and predicting prognosis in nasopharyngeal cancer and might be the biology marker for determining high risk individual ..4. The positive expression of P53 in nasopharyngeal carcinoma tissue is much higher, but in non-tumour nasopharyngeal tissue is much lower. p53 is one of the factors related to the tumorigenesis and development of nasopharyngeal carcinoma.. And it can be used as one of the factor to determine the NPC.5. The expression of the COX-2, EGFR,Ki67 and p53 can not be used as useful indicators for determining the existence of the lymphatic metastasis.
