Objective: To analyse the alterative characteristic of the extravascular lung water (EVLW) in the patients with septic shock, attempt to clarify its value on the prognosis of these patients, and try to find an objective and clinical assessment of the capillary leakage syndrome. Methods By the methods of forward-looking design and retrospective analysis, according to the ultimate survival, 20 patients with septic shock were divided into two groups. By observing the clinical features of the patients and using of a PICCO monitor for hemodynamic monitoring, mensurate the EVLW and other hemodynamic indicators, and also analyse the relationship between the EVLW and the prognosis of patients. Results On the first day, both of the two groups'EVLW were significantly increased (the means were 19.9±8.0ml / kg and 18.7±8.1 ml / kg respectively), but without any statistical difference between them. However, on the second day, after the intensive therapy, the EVLW of the survival group dropped significantly, but the one for the death group only declined slightly, and compared with the result of the first day, there was no obvious difference. The difference between these two groups was distinct (the means were 13.8±4.5 and 18.6±6.3 respectively, P <0.05). After the third day, the difference between them was more remarkable.Conclusion⒈the EVLW in the patients with septic shock increases significantly.⒉the dynamic changes of the EVLW could be assessed for evaluating the effectiveness of the treatment and predicting the prognosis of the patients with septic shock. At the same time, it also prompts that EVLW could be an objective indicator to evaluate the pulmonary capillary leakage⒊PCT is more effective and sensitive than CRP and white blood cell counting for estimating the septic shock's prognosis.
