| Objective To compare the clinical results of wavefront-guided LASIK with iris-registration versus wavefront-guided LASIK in the treatment of myopia. Methods This was a prospective randomized study of 92 myopic eyes (46patients) that underwent LASIK .wavefront-guided LASIK with iris-registration was operated on the first eye of the patient (study group) and the other eye of the same patient was operated on with wavefront-guided LASIK (control group).Besides visual acuity and refractions ,the higher order aberrations and contrast sensitivity are also compared before and lmonth,3months after LASIK. Results The mean rotation dimensionatily of the study group was 4.88°±2.84°(0.00°-10.3°) ,Pupil Centroid shift: (0.16±0.08 ) mm (0.01-0.41) mm in the X Axis, (0.11±0.07) mm (0.00-0.41) mm in the Y Axis. The postoperative refractions were not statistically significant between the two groups .(t=0.430, P>0.05). 60.9% of the study group (28 eyes) and 50% of the control group (23 eyes) respectively,had postoperative uncorrected visual acuity than their preoperative best corrected visual acuity. Three months after LASIK, higher order aberrations (RMS in 6-mm zone) were both increased in the study and control groups .No statistically significant difference was noted between the two groups( t=0.925, P>0.05).Three months after operation, statistically significant difference was noted between the two groups in the contrast sensitivity( t=2.526, P <0.05). Conclusion: Patients undergoing wave -front-guided LASIK with iris-registration have better quality of vision after surgery than the patients undergoing wave -front-guided LASIK.
