Objective: The purpose of this study by the clinical observation to the changes of the serum CRP(C-reaction protein): To investigate the changes and significance of serum CPR in patients with ACI(acute ischemic stroke); To study the relationship between CPR and TCM syndrome differentiation-type, the degree of nervous function impairment ,the volume of infarction size; to evaluate the correlation between the serum CRP and the degree of nervous function impairment with ACL To search for some objective, quantitative indicators in the ischemic stroke diagnosis and TCM syndrome type.Methods: The serum concentrations of CRP in 107 patients with ACI and 30 healthy controls were measured respectively by immune turbidity determination method.Result: 1, compared with the control group, the serum CRP in patients with acute ischemic stroke significantly higher (P <0.05); 2, the serum levels of CRP of the classified types of TCM based on syndrome differentiation were higher than that of the control group((P <0.05); CRP level sort: the wind-heat disturbance group >the wind-phlegm accumulation group >the phlegm-heat and bowel stoppage group >the phlegm moisture disturb spirit group > the wind-phlegm and blood stasis group> the wind formation due to yin deficiency group > qi deficiency with blood stasis group, the F-test showed statistically significant difference(P <0.05); the serum levels of CRP in the wind-heat disturbance group were the highest, compared with the wind-heat disturbance and phlegm-heat and bowel stoppage group, the wind-phlegm accumulation group and so on, there was significant difference between them((P <0.05). 3. For the cumulative points for the illness condition of TCM, there were significant difference in the serum CRP levels in mild, medium, severe type and extremely severe groups, with statistical significance (P <0.05); there was not significant difference between the mild group and medium group ,the others on comparison of various types are significant differences (all P<0.05). Correlation analysis showed that the levels of serums CRP were positively correlated to The cumulative points for the illness condition of TCM(r=0.769, P<0.01); 4, for the degree of the nervous function impairment, compared with the control group, the concentrations of CRP all increased significantly in mild, medium, severe type groups(all P<0.01); and there were significant difference among mild, medium, severe type groups(all P<0.05). the levels of serums CRP were positively correlated to the degree of the nervous function impairment (r=0.552, P<0.01). 5, Remarkable difference of the serum CRP in the groups of the different infarction volume, large volume group> medium volume group > small volume group (P<0.05) .Conclusions: 1, This study showed that CRP level in patients with ACI was significantly higher than the control group (P <0.01); CRP was related to the incidence of ischemic reperfusion, inflammation involved in the pathological mechanism of cerebral damage after ischemic stroke. 2. the serum levels of CRP of the classified types of TCM based on syndrome differentiation were higher than that of the control group((P <0.05); the serum levels of CRP in the wind-heat disturbance group were the highest, compared with the other group , there was significant difference between them((P <0.05). wind-heat disturbance interference on the inflammatory response even more serious. CRP may be related to the fire of evil in the medical-related. 3, the levels of the serum CRP increased with the increase of the degree of the nervous function impairment ,the volume of infarction, the points for the illness condition of TCM; the serum CRP might be the important parameters for clinical evaluation of severity in ACI.
