Objective To investigate the change in T-lymphocyte subset and natural kill cell counts after autologous blood transfusion,on the basis of which to evaluate the protective effect on cytoimmunity by ligustrazine,methods Forty ASAâ… -â…¡patients undergoing elective lumbar surgery were randomly allocated to either ligustrazine autotransfusion group(study group)or autotransfusion group(control group).The patients without any blood and endocrine system diseases evaluated bleeding over 500ml were selected for the investigation before surgery.In study group,ligustrazine with the dose of 4mg/kg had been injected intravenously to the patients 30min before their blood was collected.And ligustrazine was put into the washing saline and heparinized saline solution up to the concentration of 0.05mg/ml.Shed blood of both groups were collected and heparinized with the same autotransfusion device(Medtronic autolog).The red cells were filtrated, separated and washed before reinfusion to the patients,the salvaged blood samples were measured before reinfusion,venous blood samples were taken before surgery and on 1hr,1st5th day postoperration for the determination of T-lymphocyte subset(CD3,CD4,CD8) and natural kill cell(CD16+56)counts by flow cytometry and count CD4/CD8.As the negative control,no ligustrazine was put into the washing saline and heparinized saline,at the same time,no ligustrazine was injected to the patients.Results There was no difference on age,sex,weight,blood loss,the volume of reinfusion between the two groups. The level of CD3,CD4,CD8 had no difference in two groups.The level of CD3 decreased obviously on 1hr,1st day of postoperation compared with preoperation in both groups,with contral group decreasing more obviously than study group at all point time,the same as in 5th day of postoperation in contral group but recoverd normal in study group.The level of CD4 decreased obviously on 1hr,1st day of postoperation and recoverd normal on 5th day compared with preoperation in contral group,but only decreased on 1hr postoperation and recoverd normal on 1st day in study group,with contral group decreasing more obviously than study group.The level of CD8 had less difference during the operation,the same as comparing in both groups.CD4/CD8 decreased obviously on lst,5th day of postoperation compared with preoperation in contral group but less changed in study group,the difference of two groups was statistical significant.The level of CD16+56 increased obviously at 1hr postoperation,recoverd normal on 1st day postoperation and decreased obviously 5th day postoperation compared with preoperation in contral group,but increased on 1hr,1st day postoperation and remained normal compared with preoperation on 5th day in study group.Conelusion Both groups could cause a certain degree depression on cytoimmunity during intraoperative blood salvage.Ligustrazine had protective effects on red cells so that it significantly bring about less inhibitory impact on cytoimmunity.The study group have a rapid recovery of cytoimmunity compared with the control group.The application of ligustrazine in autotransfusion was effective and safe.
