Objective To explore the distribution of pathogens causing chronic suppurative otitis media (CSOM) in Xinjiang region and examine antibiotic sensitivity of certain isolated bacteria for guiding appropriate use of antibiotics.Methods Middle ear pura were collected from 151 patients (164 ears)with chronic suppurative otitis media and cultured for aerobic bacteria and fungus, and antimicrobial susceptibility test was also done.Results There were 154 strains of microbes isolated from the 164 samples(93.9%), among them,143 samples(87.2%) were aerobic bacteria and 11(6.7%) were fungus , 10 specimens had no growth (6.1%). In wich ,119 samples were occurred in Uighur ,22 were in Kazak , 20 were in Han, 2 were in Mongolian, and only 1 in Hui..Majority of the patients(31.1%)belonged to,30-39 years of age,sex differential is not significant. Organisim distribution is not significant in different nationalities.Bacterial detection rate were:Staphylococcus aureus,40 strains (24.4%)were the most common pathogen, followed by Proteus, 23 strains(14.0%),Pseudomonas aeruginosa, 19 strains(11.6%), coagulase -negative staphylococci, 16 strains ( 9.8%), Corynebacterium ,15 strains (9.1%), other 11bacterial species of rare, 30 strains for total(18.3%),and fungul growth of 11 strains(6.7%); the fungul growth of 11 strains are all belonged to Cholesteatoma,it should be considered to some extent.The bacteriological profile is to some extent dependent on the seasonal variations..The antibiotic sensitivity was variable among the isolated bacteriam stains. The antibiotic susceptibility tests demonstrated that vancomycin, gentamicin, ampicillin/sulbactam, were good in activity to gram-positive baceria,and penicillin G, ampicillin, erythromycin has significant resistance;penicillin, cefotaximeâ…¢andâ…£,imipenem and gentimicin has good susceptibility for gram-negative bacteria, where as penicillin G, cefotaximâ… andâ…¡has the considerable resistance.Conclusion Staphylococcus aureus, Proteus, Pseudomonas aeruginosa and coagulase-negative staphylococci are the main pathogenic bacteria of chronic suppurative otitis media. Although the dry climate of Xinjiang region is not benefit for fungal growth, according to results of this study, the fungal infection can not be ignored. Sensitivity test has significant role on guiding appropriate use of antibiotics,which may shorten the course,reduce the cost,relieve the pain and reduce the incidense of intracranial and extracranial complications.
