Analysis Of 272 Cases With Fever Of Unkown Origin

Objective:to investigate the etiology,diagnostic methods and procedures for management of fever of unknown origin(FUO) in respirator?,department.Methods: review the medical records retrospectly.summarise and analyze all the 272 patients with fever admitted to our department between 2002 to 2006.Results:(1)117(43.01%) cases among all the 272 cases consisted with FUO,155(56.99%) cases just had fever and did not consist with FUO.(2)102(87.18%) cases among the 117 cases had a definite diagnosis,and 77(79.38%) cases were infectious disease,13(13.40%) cases were collagen vascular disease,and 9(9.28%) cases were tumor and malignant diseases.15 (17.09%) cases were undiagnosed.Among the 155 cases,148(95.48%) cases were infectious disease,4(3.48%) cases were collagen vascular disease,and 3(1.94%) cases were other diseases,no tumor and malignant diseases,5(3.23%) cases were undiagnosed.(3)Males:40 cases(72.73%) were infectious disease,6 cases(10.91%) were tumor and malignant diseases,3 cases(5.45%) were collagen vascular disease. Females:37 cases(59.68%) were infectious disease,10 cases(16.13%)collagen vascular disease,3 cases(4.84%) were tumor and malignant diseases.(4) Among the different age,infectious diseases were the first etiology,in the group of aged between 21-30 years,30-40 years collagen vascular diseases were the second etiology,in the group of aged＞60 years tumor and malignant diseases were the second etiology.(5)In the cases with duration of＜4 weeks,the main cause of FUO was infection diseases,but with the duration being longer(＞4 weeks),the ratio of tumor became increasing.(6)Diagnostic methods:diagnostic therapy performed in 33 cases(32.35%),imageology test performed in 29 cases(28.43%),bacteriology test performed in 15 cases(14.71%),immune test performed in 12(11.76%) cases,biopsy performed in 8 cases(7.84%),and marrow test performed in 5 cases(4.90%).Conclusions:Most of the FUO can be diagnosed by detailed examination and analysis.Infectious disease,collagen vascular disease and malignant rumor were the major causes of FUO in this group.
