Object:To estimate the safe in the induced hypotension with nitroprusside by the change of regional cerebral oxygen saturation(rSO2) in young and old age-group.Methods:Two groups were selected in the study, one of which was 18~30 years old and the other was 60~85 years old.Their rSO2,arterial blood pressure(ABP),cardiac output(CO)were continuously monitored and recorded during perioperative period. To observe the change of rSO2 in the induced hypotension with nitroprusside. Results:MAP was different from before and after induced hypotension; There was a significant difference in older age-group in rSO2 from before and after induced hypotension, the change in young age-group was tiny; The CO of the young group was different from before and after induced hypotension; The parameter of blood gas had no difference from before and after induced. Conclusions:The rSO2 monitoring in the induced hypotension with nitroprusside is significant to cerebral oxygen balance. The patient of young age-group is safty in the range of MAP decreased 30 percent than base pressure level, but the patient of old age-grounp need promptly and accurate method to monitor the change cerebral oxygen balance to estimate the safe. OBJECT:To investigate the management of cerebral oxygen balance during neck regional main artery surgery by the monitoring of near-infrared spectrometer.METHODS:Twenty patients(ten with carotid artery stenosis, ten with carotid body tumor)were enrolled in the study, their rSO2,artery blood pressure and carotid reverse blood pressure were continuously monitored and recorded during perioperative period.RESULTS:There was a significant difference in rSO2 between two side rSO2 monitoring; The rSO2 of the patients with carotid body tumor during carotid clamping was different from before and after clamped carotid; The short time change of rSO2 of patients with carotid artery stenosis between before clamped and after clamped carotid is different. Two patients underwent artery bypass operation for the reflu pressure of internal carotid artery was too low.CONCLUSION:The rSO2 monitoring during neck regional main artery surgery is significant to cerebral oxygen balance.
