OBJECTIVEThe incidence of polypoid lesions of gallbladder shows a rising trend year by year. It has become a surgical common disease in the clinic. It was divided into tumor and non-tumor two categories in pathology . But it is very difficult to distinct the nature of the disease in the preoperative . how to handle and deal with polypoid lesions of the gallbladder, when treatment decisions were made are both important issues. To detect the serum proteomic patterns in tumor-like polypoid lesions of the gallbladder patients by surface enhanced laserdesorption/ionization time of flight mass spectrometry(SELD-TOF-MS), and establish diagnostic model.To study the clinical significance of the diagnostic model in the diagnosis and differential diagnosis for the tumor-like polypoid lesions of the gallbladder.METHODSSurface enhanced laser desorption/ionization time of flight mass spectrometry(SELDI-TOF-MS) technique and WCX Magnetic Beads were used to detect the serum proteomic patterns of 23 patients with tumor-like PLG, 21 patients with non tumor-like PLG and 26 normal persons. Biomarker Wizard and Biomarker Patterns Software were used in combination to analyze the data.RESULTSPreliminary screening out 22 representative specific proteins for the diagnosis of the tumor-like PLG. Analysis system under the conditions set selected 3 specific proteins to establish diagnostic model for the tumor-like PLG. The sensitivity and specificity of the model for the diagnosis of the tumor-like PLG were 100% and 89.4%, respectively. CONCLUSIONSELDI-TOF-MS technology can select specificity protein in tumor-like PLG; Analysis system under the conditions set selected three specific proteins to establish diagnostic model for the tumor-like PLG. Their Molecular Weights are 5064.52Da,7916.33 Da and 7643.32 Da, respectively. The model can be used for the preoperative diagnosis of PLG. This can clear the nature of the polypoid lesions of gallbladder. It can be used for the differential diagnosis between the tumor-like and non-tumor-like polypoid lesions of gallbladder. It can guide the clinical treatment; We do not know the proteins that we selected. That needs to be further studied.
