Abstract/Summary:
PrefaceIgA nephropathy(IgAN)is a name based on immunopathology,immunopathology consequence of renal biopsy is IgA depositing on glomerular mesangium of bead; characteristic of clinica is glomerulonephritis representatived by hematuria and proteinuria.Some systemic disease such as anaphylactoid purpura and cirrhosis can result to IgA deposited on glomerular mesangium.Renal injury caused by mycobacterium tuberculosis includes clinical tuberculosis of kidney,patho-renal tuberculosis,allergy syndrome of tubercle.However,MTB being cultured from urine needs 4 weeks and discharging to urine of tubercle is intermittency,so bacterium negative tubercle is 70%of the sum total.Early diagnosis,prevention and cure were extremely frail to the kidney injury correlated to mycobacterium tuberculosis.Purpose of this test is making sure that mycobacterium tuberculosis Ag85 is useful to the early diagnosis of kidney injury caused by mycobacterium tuberculosis.Detecting MTB-Ag85 and IgA1 on the renal biopsy tissue of patients who infected mycobacterium tuberculosis to confirm the dependability of mycobacterium tuberculosis and IgAN.Materials and Methods1.Materials(1)Subject and group:①Positive control group:chinical renal tuberculosis nephridial tissue paraffin section and 24hs urinary sediment. ②Control group:kidney tissue and 24hs urinary sediment of primarily nephritis patients from uropoiesis internal medicine without tuberculosis.③Experimental group:kidney tissue and 24hs urinary sediment of tuberculosis patients from uropoiesis internal medicine.(2)Archi- reductant and medicine:Anti Mycobacterium tuberculosis Ag85; Anti-Human IgA1-FITC;SABC kit.(3)Archi-instrument:37℃incubation,microwave oven,light microscope, fluorescence microscope,photomicrograph photoinstrumentation,et,al.2.Methods(1)Mycobacterium tuberculosis cultured from urine.(2)Nephridial tissue checking.(3)analysis of clinical data:PPD test,hemo tubercular antibody test.(4)Immunohistochemistry consequence analysis:Expression of MTB-Ag85 were examined by immunohistochemistry.Five high power fields(400×)were chosed randomly in every slice and measured the value of Intergratede optional density(IOD) using microimage analyzer under light microscope.(5)Iimmunofluorescence consequence analysis:Expression of IgA1 were examined by immunofluorescence.Five high power fields(400×)were chosed randomly in every slice and measured the average gray value.(6)Statistical analysis method:All data were hadled by SPSS13.0 Metering data between two group were tested by x~2 test.Results1.Mycobacterium tuberculosis cultured from urine:Mycobacterium tuberculosis masculine is 3 of the 10 patients of the positive control. Mycobacterium tuberculosis masculine is 7 of the 42 patients of the experimental group.None of the control group.2.Nephridial tissue pathology detection:(1)Experimental group renal biopsy pathological diagnosis consequence:26 pathocategory were IgAN.(2)Consequence of immunohistochemistry to detect MTB-Ag85:There were 27 masculine in the experimental group.The IOD value of positive control group (36.54±10.06)×10~3 comparing to experimental group IOD(33.69±11.82)×10~3 (P>0.05).(3)Consequence of immunofluorescence IgA1:There 35 tissue slice were detectd to IgA1 in the experimental group nephridial.The average gray value of positive control group,negative control group and experimental group is respectively(59.83±4.16),(10.02±1.38)and(57.94±4.08).The statistics discrepancy existed between negative control group and experimental group(P<0.05).3.There exists pertinency between:Ag-85 test,PPD test and hemo tubercular antibody test.Conclusions1.MTB-Ag85 on renal tissu can be early diadynamic criteria to tuberculosis infection correlated renal injury.2.The dependability existes between the renal injury caused by mycobacterial infection and the morbility of IgAN.
