Object Approach the role of TNF-αin the development of Idiopathic Pulmonary Fibrosis and the high level of TNF-αstands of the activity of IPF. Method 20 IPF patients and 8 healthy volunteers were chose randomly as IPF group and control group.Before the IPF patients accept treatment of glucocorticoid and immunosuppressive agent, we draw their blood at morning when the IPF patients were empty stomach.We draw the volunteers'blood at morning when they were empty stomach as well.To compare the difference of TNF-αbetween the IPF group and the control group.Result The level of TNF-αin the IPF patients'serum is higher than the volunteers'serum. Conclusion In the term of HRCT and the pulmonary function test,the high level of TNF-αhints that the patient was the condition of IPF.Object Approach theⅡartificial recombinate receptor antibody fusion protein of tumor necrosis factor could inhibit the development of pulmonary fibrosis, and the different role between the Etarnecept and dexamethasone to inhibit the development of pulmonary interstitial fibrosis.Method Inject bleomycin into the tracheal to make the pulmonary fibrosis models,then divided the rats into three groups,the bleomycin group(positive control group),the Etarnecept group and the dexamethasone group,and every group has 24 rats. 0.2 mL sodium chloride was injected into the other 24 rats tracheals that were included in the negative control group. We inject 0.5 mL sodium chloride into the bleomycin group rats'abdominal cavities per day,to the rat in the Etanercept group,we inject 0.4 mg Etanercept per kilogram into their abdominal cavities twice a week,and 3 mg dexamethasone per kilogram into the dexamethasone group'abdominal cavities every day.Kill 6 rats of every group on the 7th ,21st ,28th ,42nd days specially.Then we detect the level of Hyaluronic acid andⅢprecollagenous in the rats'blood serum in the 7th ,21st ,28th ,42nd days specially and observe the degree of alveolitis and the pulmonary interstitial fibrosis on the sheet stained by Hematoxylin Eosin and Masson.We observe the different role of Etarnecept and dexamethasone to the development of pulmonary fibrosis.Result The level of Hyaluronic acid andⅢPrecollagenous in the bleomycin group rats'serum is higher than the negative control group,p<0.01,and the statistical signidicance is conspicuous. The level of the Hyaluronic acid andⅢprecollagenous in the Etanercept's group is lower than that in the bleomycin group's and dexamethasone group's, P<0.01 besides P<0.05 on the 28th and the 42nd day,these results are statistical significantly, compare the pulmonary tissues'histopathology of the other three group rats from the HE and Masson stained sheet,the difference is significant.Above all of these,we can conclude that Etarnecept could inhibit the development of pulmonary fibrosis effectively, the fatality rate of the disease is degraded effectively as well.We discover that Etarnecept may induce lung abscess,and we need the other experiments to vertify the fact.Dexamethasone can inhibit the development of pulmonary fibrosis as well,but through our observation, we conclude that the effect of dexamethasone is weaker than Etarnecept.Conclusion Bleomycin can replicate pulmonary fibrosis successfully,and the operation is easily.We could evaluate the degree of the disease by detect the level of HA and PC-Ⅲ.Etarnecept and dexamethasone can inhibit the development of pulmonary fibrosis,and the effect of Etarnecept is more significant than dexamethasone.Etarnecept maybe a new therapy to pulmonary fibrosis.
