【background】In the early stage of severe burn, effective anti-shock treatment is the key point to influence the progress of the whole state of the burn . So far, however, infusion treatment is the only effective intervention measure aimed at vasopermeability, which is a research hot spot to cure early stage severe burn. Vascular endothelium cells are the main barrier which constitutes the vasopermeability. A lot of prospective research showed that early insulin intervention had obvious protective effects on severe patients including severe burn patients. Although the mechanism is still unclear, insulin's cell protective effects have been gradually realized. There are no reports on insulin's effects on anti-effusion and anti-shock and whether it has protective effect on endothelium cells. 6 【Purposes】1,To interpret whether insulin treatment can lighten the lung effusion and the damage of endothelium cells in animal in vivo test.2,Preliminary analyze the possible signal transduction pathway of insulin's anti-apoptosis of endothelium cells in vivo and in vitro.【Contents】1,The protective effect of insulin on the acute lung damage of severely scalded rats.2,The protective effect of insulin on early endothelium cells of severely scalded rats.3,The protective effect and mechanism of insulin on endothelium cells apoptosis with the burn blood serum induction.【Results】1,Burn early lung effusion can be lightened and the structure and function of lung can be effectively protected after the disposal of insulin.2,The disposal of insulin could significantly depress the damage of endothelium cells caused by severe burn and enhance the level of NO and eNOS.3,In vitro experiments proved that insulin could depress the apoptosis of endothelium cells stimulated by burn blood serum, and increase the level of NO.4,In vitro experiments showed that insulin increase the production of NO by activating Akt-eNOS signal transduction pathway, which may be the mechanism that insulin depress the apoptosis of endothelium cells after burn.【Conclusions】1,Insulin intervention could effectively lighten acute lung damage in early stage of severe burn.2,Insulin had protective effects of anti-apoptosis of endothelium cells in the stimulating of burn, and it can play a key role in anti-effusion and the protection of organs in early stage of burn.3,Insulin increase the production of NO by activating Akt-eNOS signal transduction pathway, which may be the mechanism that insulin depress the apoptosis of endothelium cells after burn.
