Objective:With the increasing number of hydrothorax and ascites patients,how to improve the accuracy of cytological diagnosis of pleural effusion and ascites had been concerned by more and more people.However,the positive rate of cytological diagnosis had been influenced by many factors,and its specificity and sensitivity was not very high.To improve positive rate effectively and reduce the rate of misdiagnosis and missed diagnosis,our experiment carried on an investigation to these factors which influenced accuracy of cytological diagnosis and found fundamental influential factors.The best combination of these factors would be discussed at last.Method:Patients came from the tumor medicine,respiratory medicine and digestive medicine in the First Affiliated Hospital of Anhui Medical University.From October 2006 to April 2007,hydrothorax and ascites samples of patients were collected.With the self-statistical table,the data was collected.The study included that based on single factor experiment,specimen volume,the frequency of inspection and the diagnosis of the original sites were analyzed.In order to achieve best combination,the inspection conditions of cytology were optimized with orthogonal experiment.Results:1.Until April,2007,the total number of the samples was 187,which came from the tumor medicine,respiratory medicine and digestive medicine in the First Affiliated Hospital of Anhui Medical University.Among the samples,the total number of pleural effusion samples were 117 and the total number of ascites samples were 70.2.After single factor experimental analysis,the level of the fundamental influential factors of cytological diagnosis was that the sample was submitted for once,twice and three times;20 ml,40ml,80ml samples were examined and the original sites were not obvious or clinical negative diagnosis or clinical positive diagnosis.3.The ultimate results show that in the conditions of clinical positive diagnosis,the inspection of 40ml sample was examined for twice,the positive rate of examination is 72%,which was higher than the results of examination by traditional method and the same as the results of the theoretical level group.Conclusion:1.The positive rate of cytological examination on pleural effusion or ascites was closely related to specimen volume,the frequency of examination and the diagnosis of the original sites.2.After single factor analysis for the level of influential factors,the results of optimized combination on the different levels of these factors with orthogonal experiment could play a role in improving positive rate and reducing the rate of misdiagnosis or missed diagnosis,and had a guidance on clinical diagnosis and therapy.
