Objective To analyze the galactographic features in breast cancer with nipple discharge and the correlation between the galactographic and pathologic findings. To evaluate the value of galactography that diagnosis breast cancer with pathologic nipple discharge.Methods Pathology were proven in 23 cases of breast cancer presenting pathologic nipple discharge,including 18 cases of nonpalpable breast cancer and 5cases palpable breast cancer . To compare breast cancer with 48 cases of intraductal occupied whoes includes 36 cases of Intraductal papilloma (benign lesions )and 12 cases of papillomatosis(precancerous lesions). A retrospective study was performed to analyze the galactographic features included the location of lesions, characteristic of intraductal filling defect and ductal obstruction, and to correlation the galactographic and pathologic findings . To evaluate the diagnostic and different diagnostic value of galactography that diagnosis breast cancer with pathologic nipple discharge.Results⑴Galactographic features of breast cancer:18 cases presenting nipple discharge breast cancer'galactographic features were mostly irregular intraductal defect with ductal wall breakage and ductal obstruction in the peripheral ducts, Including 2 cases with multiform clustered microcalcifications pervadingly indicates breast cancer, diffuse fill by tumour encroach duct have characteristic. In addition, 5 cases palpable breast cancer galactographic features were irregular foliar mass and dim border with specula,and tutor crush duct result in ductal obstruction.⑵Galactographic features of intraductal papilloma:36 cases of intraductal papilloma presents smooth intraductal filling defect with ductal wall integrity and ductal dilatation,torsion.⑶Galactographic features of papillomatosis (precancerous lesions ):Papillomatosis presents small multiintraductal filling defec in the peripheral ducts mostly.⑷The location of lesions: Of the 23 breast cancer cases, 16(69.6%)cases had lesions in the subsegmental duct and its branchesand ,7(30.4%)cases had lesions located in the main mammary ducts. Of 36 cases intraductal papilloma, 5(13.9%) cases in the peripheral mammary duct and 31 (83.1%) cases were located in the main mammary duct. Of the 12 papillomatosis cases, 10 (83.3%) cases in the peripheral peripheral mammary duct and 2 (16.7%) cases were located in the main mammary duct.⑸The accuracy rates of galactography for breast cancer with nipple discharge,intraductal papilloma and papillomatosis were 82.6%,83.3%和75.0%, respectively.⑹Kinds of pathology:cases of 11 infiltrative ductal carcinoma,8 cases intraductal carcinoma,3 cases papillary carcinoma and 1 case lobular carcinoma in situ .In breast cancer with nipple discharge, early breast cancer occupy multitud(e52.2%) especially in breast cancer without palpable mass.Conclusion Breast cancer with nipple discharge have some diagnostic galactography findings , galactography is a useful and accurate in the diagnostic workup of discharging breast, especially in differentiating between the benign or malignant lesions in patients with spontaneous nipple discharge. In addition, galactography also accurately diagnostic early breast cancer without palpable mass.
